Position Summary:

The Health Coach is responsible for the provision and/or oversight of wellness care for Coryell Health Medical Clinic patients in accordance with the Texas Nursing Practice Act. This person is responsible for overseeing and coordinating health services with members of the CH team and other community providers in order to provide preventative health services routinely.

Qualifications:

1. Currently licensed as a Licensed Vocational Nurse, CNA or Medical Assistant in the State of Texas. Certified health educators may also be considered with an associate’s or bachelor’s degree.
2. Prefer two years of experience in a clinical setting.
3. Must be able to work independently and part of a team, possess strong organizational and time management skills and excellent verbal and written communication skills.
4. Must have knowledge of standard record keeping procedures (i.e. Progress notes, HIPPA guidelines).
5. Must be able to facilitate communication and problem solving with patients, families, staff physicians and administration.

Duties and Responsibilities:

• Provide daily outreach to a designated population in order to initiate annual wellness visit appointments.
• Offer regular annual wellness visits to qualifying patients and provide the following services as appropriate:
• Preventative direct care interventions such as blood pressures checks, weight and vision checks, depression screening, fall risk screening and short portable mental status questionnaire.
• Review medications, update medication record as necessary, communicate any discrepancies or concerns regarding medication regiment with patient and with PCP as deemed necessary.
• Review of patient’s social, medical, surgical, and family history as necessary, update patient’s chart and report any discrepancies or concerns to PCP.
• Review of all preventative exams appropriate to patient and determine a written schedule for provider to review and determine what referrals or orders are needed. This includes lab work, immunizations and diagnostic screenings.
• Provide information and education on health issues, stressing the importance of participation in preventative activities, evidence based practices and the active self-management of chronic conditions.
• Obtain appropriate specialty records on preventative care for PCP to review as deemed appropriate.
• Complete follow-up on all ordered testing/referrals to ensure that the orders were completed and to obtain records for patient’s PCP to review.
• Complete appropriate computer documentation to maintain patient’s medical record and initiate provider’s documentation with the data collected by the wellness nurse.
• Updates each patient’s health maintenance module during annual wellness visits and as preventative testing/referrals are completed.
• Utilize health maintenance to identify patients that are due for individual preventative measures. Provide this information to PCPs on a daily basis to help close care gaps on patients being seen that same day.
• Other duties as assigned.

Job Type: Full-time

Pay: $13.00 - $17.00 per hour
