Summary:
The HIV Outreach Case worker will be responsible for coordinating and implement the Outreach Activities for the HIV/AIDS program in the service area. The Outreach worker will conduct outreach activities to targeted persons at risk for HIV/AIDS and other STD’s and help link them to Borrego’s comprehensive health services. Outreach includes ongoing assessments of services needs, linkage to resources, planning to obtain identified services/resources, monitoring, client-specific advocacy, and coordination of care and services with other team members. Uses analytical skills to provide continued program improvement.
Essential Duties and Responsibilities:
Administrative Functions:
1. Actively maintain a presence in the community.
2. Provide professional intensive outreach by connecting with service providers and medical professionals to identify out of care HIV positive individuals and link to care.
3. Develops appropriate workflows acceptable to the project and BCHF administration.
4. Ensures compliance with county, state and federal regulations within the assigned programs, including but not limited to Department of Health Services Licensing.
5. Conduct outreach/social marketing in the community as well as reach out to community providers to assist in locating clients who are out of care.
6. Administer pre/post testing and comfortable with administrating HIV testing.
7. Maintain database of patient encounters and outreach data.
8. Work with the local Health Department and Epidemiological staff to identify target demographics, as well as the targeted geographical service area.
9. Complete and submits all required monthly progress/program reports.
10. Works outside regularly scheduled work hours as needed including evenings and weekends.
11. Engage with community advisory groups PLWHA’s to further assist in determining and establishing where service shortcomings are, why they exists, why clients are not in care, and why clients who were previously in care dropped out.
12. Any other duties as may be assigned by the immediate supervisor.
13. Is fully knowledgeable of the details of the grant, program or project and is able to closely evaluate and monitor performance at all levels.
14. Integrates support functions effectively with the clinic services and operations.
15. Ensures compliance with county, state and federal regulations within the assigned programs, including but not limited to Department of Health Services Licensing.
16. Keeps abreast of current related issues through committee meeting participation, community events, networking, electronic media or other methods.
