PURPOSE:
The Home & Community-Based Services Manager is responsible for leading and managing the DHS-funded Home-Based Supports Services and Respite programs and services, which serves children and adults who have disabilities and their families under the DHS Home-Based Waiver and DHS Respite Grant. The position is also responsible for directly supervising all program staff and adhering to DHS rules and regulations for the program.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
The list of essential duties and responsibilities, as outlined herein, is intended to be representative of the task to be performed. The omission of an essential function does not preclude management from assigning duties not listed herein is such duties are a logical assignment to the position.
1. Hire, train, and provide direct supervision to Home-Based Service Facilitators, Respite Workers who provide service facilitation (AKA Self-Directed Assistance) for families under the Home-Based waiver and respite services under the Respite grant.
2. Review programmatic billing to ensure all billable hours are captured by team and provide coaching and training to work more efficiently to fulfill maximum billable service needs and programmatic financial sustainability.
3. Facilitate Home-Based and Respite Intake and Discharge Processes that includes receiving and processing referrals, checking eligibility, inputting data into database, ensure all necessary forms are completed, etc.
4. Carry a small service facilitation (self-directed assistance) caseload and provide case coordination for families served by the program when needed.
5. Participate in Home-Based Person-Centered Plan (PCP) (in home or at the office) reviews and update the PCPs as needed along with training direct reports in developing effective implementation strategies.
6. Review and track direct reports PCP and implementation strategies to ensure DHS compliance.
7. Ensure Home-Based families are receiving the desired allotment of service hours as dictated by the Service Agreement.
8. Review Home-Based team caseload assignment and reallocate caseloads based on programmatic and family needs.
9. Ensure direct reports maintain current required training certification requirements, and coordinate with the agency training department to address team training deficiencies.
10. Communicate with families receiving Respite services to ensure respite needs are being met and hours received are within the DHS Respite grant guidelines.
11. Submit family satisfaction surveys regarding Respite Worker performance.
12. Review Family Handbooks with families to ensure expectations are clear based on DHS requirements.
13. Ensure Respite Workers’ Paycom hours are matching up with the services rendered, and all direct reports timesheets are entered on-time and approved in Paycom.
14. Assists with the completion of the annual Respite grant application, and coordinates with agency accounting and grants teams to ensure sufficient utilization of grant funds awarded.
15. Lead regular team meetings with direct reports.
16. Ensure all direct reports have documented billable hours in the agency electronic case management system and run and analyze reports to ensure programmatic effectiveness and efficiency.
17. Review and track compliance with programmatic outcomes including CARF accreditation standards.
18. Review all UIRs and provide dispositions in a timely manner.
19. Provide training on individual and family engagement with direct reports, as needed.
20. Assist direct reports in managing individual and family crises and navigating the DHS system to ensure compliance with DHS funding sources.
21. Attend and participate in management team meetings and provides recommendations to program effectiveness and efficiencies.
22. Responsible for ensuring all staff are adhering to DHS rules and regulations.
23. Maintains annual American Heart Association certification in First Aid and CPR.
24. Successfully completes Little City Foundation required training, re-training, and any additional training as required by Little City Foundation policies and practices, or as assigned by his or her immediate supervisor.
25. Perform other duties as assigned.

A Bachelor’s degree in a human service field required. Must have 2-years’ experience working with children and/or adults with disabilities. QIDP required either prior to employment or shortly after being employed. Supervisory experience is preferred.

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:
While performing the duties of this job, the employee is subject to sitting, walking, and standing for prolonged periods; frequently grasps, lifts, holds, or feels objects; occasionally stoops, kneels, crouches, or crawls. The employee frequently is required to use manual and finger dexterity and eye-hand coordination when working with persons served and handling office, medical, or household equipment. The employee is subject to assist and lift up to 30 pounds with varying amounts of assistance on a reoccurring basis. The employee is required to have corrected vision and hearing within normal range and the ability to operate a motor vehicle. Must have the ability to perform CPR and CPI. Generally, no occupational exposure to blood, body fluids, communicable diseases or other potentially infectious substances, but may require performing unplanned Category III tasks (Category II). Typical office environment. The physical demands described here are representative of those that must be met by an employee to perform successfully the essential functions of this job.
