Purpose of the position
Under the direction and supervision of the Center Manager, the Personal Care Attendant provides personal care services to Sunrise PACE participants in the day health center or our resident house, as well as other activities and tasks as assigned to assist the primary care, therapy, and adult day health staff. The Home Health Aide is responsible for providing routine individualized healthcare for the elderly, convalescents, or persons with disabilities at the patient’s home or in a care facility. Monitors and reports changes in the health status. Will also provide assistance with the activities of daily living.
Requirements for this position
- Communicate effectively to a variety of persons, including participants, families, staff, and other agencies.
- Work effectively in a team environment and also independently with participants.
- Care for geriatric patients in a compassionate and patient manner.
- Only act within the scope of his/her authority to practice.
- Meet a standardized set of competencies for the specific position description established by Sunrise PACE and approved regulatory agencies before working independently.
- Conduct daily physical activities in assisting frail elders in ergonomic manner to avoid injury.
- Receive and carry out orders from Interdisciplinary Team regarding personal care services and/or monitoring of participant status.
- Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
- Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.
- High school diploma or equivalent.
- CNA license preferred but not required.
- Minimum 6 months of clinical experience as a Personal Care Aide, Certified Nursing Assistant, or EMT.
- 1 year experience with the frail or elderly population or, in the absence of such experience, to receive appropriate training from the Sunrise PACE on working with a frail or elderly population upon hiring.
- Must complete and pass the Sunrise PACE approved curriculum for Personal Care Aides upon hire.
- Geriatric population with physical disabilities and limitations.
- Proper body mechanics during provision of assistance with Activities of Daily Living.
- Safe and sanitary personal care techniques for elderly population with physical limitations.

- Physical demands: Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.
- Work Environment: Employee must be able to work in a busy and demanding clinic environment with moderate to loud noise levels.
