Position Summary:
The Registered Nurse will provide skilled nursing care to patients requiring professional nursing services in accordance with physician's orders and in compliance with state and federal regulations. The position will provide supervision to clinical staff in the provision of care to patients.

Position Duties:
- Perform and complete nursing assessments
- Develop individualized plan of care based on assessment findings, supporting documentation and physician’s orders for each patient
- Evaluate the learning ability and knowledge of the patient and caregiver to determine appropriate education
- Evaluate the patient’s environment to determine assistance needed
- Complete OASIS assessments
Other Requirements:
- Current automobile liability insurance according to company policy
- Travel required
- Must be able to speak and write in English
Licensing/Certification/Education Requirements:
- Graduate of an accredited School of Nursing
- BSN preferred
- One (1) year experience as a registered nurse preferred
- Acute care experience with the adult population in a medical-surgical environment
- Previous home health or hospice experience preferred
- Current state registered nurse license
- Current CPR certification
- Current state driver’s license
Physical Requirements:
- Audio-Visual: Hearing: Good
- Ability to lift to 40 pounds
- Work requires walking, standing, bending, reaching, kneeling, pushing and pulling
