Requirements

  • Graduate of an approved nursing program/school is required. Bachelor's Degree, preferred.
  • At least 1 year of recent acute care experience in an institutional setting is required. At least 1 year of recent experience in Home Care is required.
  • At least 1 year of Hospice experience is preferred.
  • Active Michigan RN License is required.
  • Active AHA BLS Certification is required.
  • Previous supervisory/management experience is preferred.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
  • Must possess a valid driver's license and acceptable driving record

Responsibilities

  • Receives case referrals, reviews available patient information related to case and assigns appropriate hospice personnel to case as needed.
  • Reviews and evaluates each case through home visits, conferences, record review and instructs and guides clinicians to promote more effective performance and delivery of quality services.
  • Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities and in developing plan of care.
  • Attends IDG/IDT meetings with hospice personnel to facilitate coordination of care and discussion
  • Assists in the screening and interviewing process of applicants and makes recommendations for employment. Assists in the orientation of new employees.
  • Assesses the health needs of the community and assists in the development of plans/programs to meet needs. Participates in public relation and community activities that promote the organization's role as an effective member of the hospice and health care deliver system.
  • Other duties as assigned.
