Housing Case Manager
Job description
Under the direct supervision of the Lead Housing Case Manager or Director of Client Services for Baltimore City, the Housing Case Manager will provide direct care services and group programming dedicated to youth and young adults ages 18-24. Programming will be geared towards promoting self-advocacy in the attainment of their own, client driven life goals that will include but is no limited to housing stability, physical and emotional wellness, education and career enhancement. As a Housing Case Manager, this position will be responsible for working with homeless or unstably housed youth to attain housing utilizing rapid rehousing funds and provide ongoing case management services to ensure long term self-sufficiency. In this role, the Housing case manager will work to develop partnerships with landlords and property managers that increase youth ability to access safe affordable housing. Under the supervision of the Lead Housing Case Manager or Director of Client Services, additional youth focused activities and services will be identified, scheduled and supported onsite at the Baltimore City Office.
PRIMARY DUTIES AND RESPONSIBILITIES
· Provide rapid rehousing and ongoing case management services to 14 transitional aged and/or parenting youth.
· Provide an initial psychosocial assessment for each client in order to identify client’s needs, strengths, and other relevant information to provide a comprehensive assessment that address a multitude of needs such as physical health, mental health, educational and career goals.
· Provide referrals for community resources and programs such as primary care, behavioral health, substance abuse treatment, workforce development programs and other client identified programs or areas of need/interest.
· Complete an initial service plan that identifies short-and long-term treatment goals, and then update monthly to document progress and continued needs. Review with client as developed and updated.
· Provide weekly home visits to discuss housing maintenance and stability in areas including rental payments and interaction with landlord and neighbors.
· Provide onsite programs that promotes financial literacy, physical and mental wellness, healthy relationships, social behavior and risk taking analysis and conflict resolution skills.
· Participate in scheduled weekly team meetings which includes the director of client services, case manager, therapist, and other key partners to discuss youth, progress and challenges.
· Attend monthly supervision with the Director of Baltimore City Services and the Chief Program Officer to assess program goals, measure success and strategize for ongoing enhancements to ensure we are meeting the needs of our clients.
· Other duties as assigned.
GENERAL DUTIES
· Provide RRH and direct care services to transitional aged youth ages 18-24 with a history of emotional and or behavioral difficulties.
· Responsible for acting as a liaison for youth and landlords as they navigate the housing search, identification, unit selection process and assisting in any ongoing mediation or disputes to ensure youth remain stably housed.
· Effectively use HUD tools and ensure compliance with data standards and record keeping as required by HUD.
· Perform certifications, annual re-certifications and housing quality inspections for each client and accurately update files as necessary.
· Responsible for designing and implementing group program for youth at the Baltimore City Office.
· Maintain statistics regarding service delivery and other services provided to vulnerable families and individuals as necessary.
· Maintain awareness of community resources.
· Manage tasks as assigned.
SUPERVISORY RESPONSIBILITIES
None
EDUCATION and/or EXPERIENCE
· Bachelors Required, Masters in Social Work or Human Services Preferred.
· A minimum of two years experience working with youth ages 18-24 as a resource coordinator, case manager, counselor, therapist, guidance counselor, social worker, or other similar position;
· Knowledge of and ability to work with persons of diverse backgrounds.
· Knowledge of crisis counseling skills.
LANGUAGE SKILLS
Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients, collaborative agencies or employees of the Agency.
MATHEMATICAL SKILLS
Basic math skills of addition, subtraction, multiplication and division are required.

Pay: $22.00 - $25.00 per hour
