- Current Verifiable Driver's License (required)
- Auto Insurance (required)
- Current TB test (required)
- HCA registered (required) Dept. of Social Services, Home Care Aide Registry ( to include Live Scan Fingerprinting- DOJ and FBI screening)
- Copy of COVID-19 Vaccination (required)
