Bloom Healthcare is looking to add an Intake Coordinator to our team! This position will be responsible for handling new patients from the time of first contact until the first appointment with a provider. We’re looking for passionate individuals committed to improving the lives of homebound seniors. The position will report to our Director of Operations, and collaborates with our administrative team, providers, intake, and our community care partners.
Intake Coordinator Responsibilities:
- Initial phone triage for patient referrals, questions, and clinical concerns.
- Building patient charts in the EMR
- Patient Record management
- Verifying insurance
- Scheduling new patient appointments
- Communicating with patients and those involved with the patient’s care about the practice
- Following up with referral source
- Tracking new patients and referrals
- Back office tasks
Intake Coordinator Qualifications:
- High school diploma
- Bachelor's degree preferred
- 2 years of customer service experience in an office setting
- Experience in healthcare and hospice preferred
- Efficient in Microsoft Office platforms
