MHC Healthcare is seeking an Intake Specialist to join our Work From Home (WFH) team. The Intake Specialist will be responsible for conducting intakes for AHCCCS and Sliding Fee Scale clients including both adults and children. Intakes consist of completing the core assessment, PCP notification, Coordination of Care, the demographic and the service plan. The Intake Specialist provides direct client services by conducting the intake assessment. The direct care consists of case management services, skills training, health promotion, and crisis intervention, to individuals and/or family members. MHC Healthcare is a Federally Qualified Community Health Center (FQHC), with 16 sites in Tucson and Pima County. - Bachelor’s degree in Social Service field
- Basic Life Support (BLS) certification (may be completed with MHC upon hire)
- First Aid certification (may be completed with MHC upon hire)
- Fingerprint Clearance Card through the Arizona Department of Public Safety (or ability to obtain upon hire)
- Current Arizona driver’s license with clean driving record and proof of current vehicle insurance (39-month MVR will be run by MHC)
- Master’s degree in Social Service field
- Experience with State AHCCCS Complete Care (ACC) and the Regional Behavioral Health Authority (RBHA) system
- Bilingual (English/Spanish)
This position has the following supervisory responsibility:
- There is no supervisor responsibility for this position.
- Excellent customer service, organizational, and communication skills with emphasis on responsiveness, building trust, mutual respect, and courtesy.
- Computer proficient.
- Knowledge of State ACC and RBHA requirements.
- Knowledge of current versions of Diagnostic and Statistical Manual of Mental Disorders (DSM) and International Classification of Diseases (ICD).
- Ability to complete the Behavioral Health Assessment with appropriate clinical oversight.
- Completes thorough and timely intakes on all AHCCCS clients.
- Provides clients and family members with quality and timely assessments, service planning, annual updates, primary care coordination, psychiatric coordination, and discharge and aftercare planning with a recovery model of care.
- Prepares timely and accurate documentation and maintains audit-ready client charts including:
- Core assessment with yearly updates,
- Demographic Form with yearly or bi-yearly updates,
- Service Plans with ongoing Service Plan reviews,
- Annual Assessment Updates,
- Primary care coordination as appropriate, not less than annually,
- Psychiatric assessments with yearly updates,
- Crisis plans with appropriate revisions, and
- Other forms as appropriate or requested.
- Accommodates client schedules when providing intakes and direct service.
- Meets productivity requirements while maintaining ethical encountering and billing practices.
- Participates in home-based and crisis intervention services as necessary or requested.
- Provides or arranges transportation for clients as necessary.
- Provides clients with appropriate referrals to community agencies and resources as needed or requested.
- Works collaboratively with psychiatric staff as appropriate, ensuring clients receive appropriate medication monitoring, psychiatric evaluation, and regular primary care involvement.
- Provides guidance to Recovery Coach staff as needed.
- Maintains an appropriate number of direct service hours ensuring intake information is properly documented.
- Follows professional and ethical practices when working with clients, client records, families, and community personnel.
- Maintains confidentiality of client and family information.
- Promotes trust in MHC care providers while delivering excellent customer service.
- Acts as a willing and efficient liaison between MHC Behavioral Health Services and other agencies.
- Observes applicable state and federal laws and assumes responsibility for knowledge of laws, and requests clarification if necessary.
- Participates in the Behavioral Health team meetings, client staffing, case reviews, staff development, and participates in policy and procedure development.
- Assists Behavioral Health supervisory staff with client tracking, ensuring timely follow-through and timely and accurate paperwork.
- Maintains general behavioral health and education standards and keeps abreast of behavioral health and State AHCCCS guidelines by reading and attending in-service trainings and workshops as appropriate and indicated in staff development plan.
- Uses appropriate incident reporting procedures when documenting unsafe or problematic incidents involving patients, clients and staff
