Position Summary: The Team Lead provides clinical guidance for their team.

  • IIH is a time-limited intensive family preservation intervention intended to stabilize the living arrangement, promote reunification or prevent the utilization of out-of-home therapeutic resources for the identified youth through the age of 18. This service is delivered primarily to children in their family’s home. All treatment must be directed toward the eligible recipient of in-home services. This intervention uses a team approach designed to address the identified needs of children and adolescents who are transitioning from out of home placements or are at risk of out-of-home placement and need intensive interventions to remain stable in the community. Team to family ratio shall not exceed one to eight (1 to 8) for each three-person team. This population has access to a variety of interventions twenty four (24) hours a day, seven (7) days per week by staff that will maintain contact and intervene as one (1) organizational unit. Preference for individual who has training in TFCBT or trauma informed care.

Essential Job Functions:

  • Must deliver services in various environments, such as homes, schools, detention centers and jails (state funds only), homeless shelters, street locations, etc.
  • Responsible for coordinating the initial assessment and developing the youth’s Person Centered Plan (PCP)
  • Diffuse crises, evaluate nature of crises, and intervene to reduce the likelihood of a recurrence
  • Ensure linkage to needed community services and resources
  • Arrange, coordinate, and monitor other services on behalf of the individual
  • Monitor and manage the presenting psychiatric and/or addiction symptoms
  • Provide self-help and living skills training for youth;
  • Provide or coordinate (with another licensed professional) treatment for the youth or other family members
  • Provide parenting skills training to help the family build skills for coping with the youth’s disorder
  • Work with caregivers in the implementation of home-based behavioral supports
  • Provide or arrange crisis management, intensive case management, individual and/or family therapy, substance abuse intervention, skills training, and other rehabilitative supports to prevent the need for an out-of-home, more restrictive services
  • Provide, when assigned, “first responder” crisis response on a 24/7/365 basis to recipients
  • Coordinate and conduct peer review
  • Ensure incident reports are prepared and submitted on time
  • Manage utilization review
  • Assist in preparing employee evaluations
  • Assist in training new employees
  • Complete the intensive in-home services training within the first 90 days of employment
  • Other duties as assigned

Supervisory Responsibility:

  • Ensure in-home staff provides 24 hour coverage, 7 days per week
  • Provide supervision to associate professionals, provisional licensed, and substance abuse certified staff (when appropriate) according to supervision requirements specified in 10A NCAC 27G.0104 and ASAP policy 601.1 and according to licensure and certification requirements of the appropriate discipline
  • Ensure a minimum of twelve (12) contacts occur within the first month
  • Ensure for the second and third months of Intensive In-Home services, an average of six (6) contacts per month occur
  • Ensure that, when appropriate, service frequency is titrated over the last two (2) months
  • Ensure a minimum of 2 hours per of service is provided for a billable day
  • QA staff service notes and ensure timeliness
  • Approve timesheets and billing, ensuring accuracy and timeliness

Minimum Qualifications:

  • Must be either a licensed or board eligible/provisional licensed professional (LPC, LPA, LMFT, or LCSW).
  • Must have a minimum of one (1) year documented experience with population
  • Must have a valid, unrestricted, state appropriate driver’s license

Equipment Used:

Computer, Microsoft Office Suite software (excel, word), phone, scanner, fax, cell phone.

Essential physical requirements:

  • Must possess the visual acuity to develop written correspondence and determine accuracy, neatness and thoroughness of the work assigned.
  • Must be able to physically perform the basic life operational functions of talking, hearing, sitting for long periods of time and performing repetitive motions, such as using a computer.
  • Must be able to speak audibly, write legibly and perform repetitive motions such as typing.
  • Must be able to perform light/moderate lifting and carrying, for items such as marketing materials and marketing displays.

Environment:

Work is office/community-based; travel and work within home and community settings is required
