Near North Health (NNH), a 501(c)(3) non-profit Federally Qualified Health Center (FQHC), is one of the largest providers of community-based primary health care in Chicago. We provide medical services, social services, and nutrition education to medically indigent and uninsured individuals and families.

Position Summary: Facilitate social services required by clients of NNH. Maintain active clinic caseload and collaborate with other service programs within the department, as well as, other departments of NNH. Maintain a systematized routine for client referrals to the Social Services department, including those clients who require immediate social work intervention.

Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts .

Essential Duties and Responsibilities:

  • Provide individual patients with the psychosocial support needed to cope with psychosocial stressors and/or medical issues.
  • Assist medical providers by providing supportive social work and therapeutic interventions as required.
  • Assist patient in identifying/obtaining medical resources. Help patients plan for their home needs by assisting the patient with arranging at-home services.
  • Conduct mental health assessments for referred NNH patients, including EIS.
  • Provide individual, group counseling and/or family counseling for patients.
  • Ensure and meet all Domestic Violence program goals, objectives and activities (if applicable to role) in accordance with NNH Mission and Health Care Plan and applicable funding requirements.
  • Provide client education through training seminars or workshop based on client and/or community need.
  • Establish, maintain and conduct a routine system for screening patients who present with mental health and/or psychosocial issues.
  • Develop a service and or treatment plan for clients that are enrolled in domestic violence or clients receiving mental health counseling.
  • Maintain an ongoing review of service plan and keeps up to date casenotes for each client enrolled in the program.
  • Advocate for patients by accessing community services and providing appropriate referrals.
  • Maintain a system of referral and follow-up to maximize communication with internal and external service providers to enhance optimal patient care and follow-up.
  • Collect and maintain statistical information required for reporting; domestic violence, and all other social service reports required on a weekly, monthly, quarterly and yearly basis. Submit reports in a timely manner.
  • Adhere to all administrative requirements related to the professional delivery of social work services, such as collecting statistics, in-house documentation requirements and other related tasks as assigned.
  • Assess and provide individual counseling for pregnant and postpartum women with depression.
  • Participate in case staffing and case conferences with the Mental Health team, Case Management, and other NNH programs/providers as appropriate
  • Develop, collaborate and build a relationship with other NNH programs and community agencies for client services.
  • Conduct community education awareness workshops regarding domestic violence with other NNH programs and other community agencies.
  • Adhere to NNH Confidentiality Policy and Procedure, and HIPAA regulations.
  • Perform other duties assigned.

Candidate Requirements:

Education: Master’s Degree in Social Work from a School of Social Work approved by the Council on Social Work Education.

Required: Current State License, or work toward Licensure requirements is desirable. Ability to speak, write and read Spanish required.

Minimum Experience:

3 years experience in the field of social work (internships qualify). Must have the ability to: take direction/supervision; willingness to ask for clarity in ambiguous directives or situations; ability to establish and maintain professional working professional working relationships with staff and other professionals; ability to provide leadership in the areas of clinical relationships with staff and other professionals; ability to conceptualize and transmit knowledge and techniques in the resolution of problems.
