Puget Sound Home Health and Hospice is a Medicare-certified, 4.5-Star rated agency that is seeking an experienced and caring full-time Licensed Practical Nurse (LPN) for our Home Health division in the greater Pierce County, Washington area!

Essential Duties

· Provide one-on-one skilled nursing care to patients in their homes (e.g., wound care, catheter care, medication administration, symptom management, etc.)

· Provide skilled observation, assessment, and evaluation

· Provide patient and family/caregiver support and education

· Practice cohesive and collaborative care with the healthcare providers

Requirements and Qualifications

· Current WA Licensed Practical Nurse (LPN), or before first day of work

· Prior Home Health and/or Hospice experience preferred

· Ability to work independently and demonstrate critical thinking

· Current BLS certification from AHA

· Be a licensed driver with an insured automobile in good working order
