Amerimed MIH provides mobile healthcare services as an adjunct to healthcare organizations including
WellBe Senior Medical in Chicago, IL. The WellBe care model is a Physician Led Advanced Practice clinician
driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly
Medicare Advantage patients. This population is typically underserved and very challenged with access to
care. To address these problems, we have elected to bring the care to the patient, instead of trying to
bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic
care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative
care, to end of life care. WellBe's physician/advanced practicing clinician led geriatric care teams’ partner
with the patient’s primary care physician to provide concierge level geriatric medical care and social
support in the home as well as delivering and coordinating across the entire care continuum. Amerimed MIH
provides supplemental clinical care through mobile Licensed Practical Nurses working directly under the
supervision of WellBe physicians and advanced practice providers in accordance with approved clinical
protocols.

GENERAL SUMMARY
Amerimed MIH Licensed Practical Nurses (LPN) are an integral part of the WellBe care team. The LPN are
responsible for delivering in-home health services and monitoring as directed by WellBe clinicians,
including assistance with care transitions and other services as needed. This candidate will help educate
unengaged patients about WellBe’s program and close gaps in communication within the interdisciplinary
care team and with other community providers.

SKILLS & COMPETENCIES
- Fosters a culture of best demonstrated practices, customer and peer service-orientation,

- Completes initial in-home assessments and on-going home visits as appropriate.
- Completes and documents thorough home safety inspection during first patient visit.
- Checks and records health vitals including weight, blood pressure, heart rate, lung sounds, and
blood sugar levels, as well as potential health challenges noted during the home visit and/or
reported by each patient visited
- Able to perform the following procedures:
- Straight catheter
- Change foleys
- Blood draws
- Acute wound care Wound cultures
- IV fluids and IV removal
- Suture/staple removal
- Retinal scans
- Administer vaccinations
- Support medication delivery
- Provide Covid/flu/strep test
- Perform the portable peripheral arterial disease test
- Perform EKGs
- Coordinates smooth transitions post discharge
- Completes medication reconciliation for TOC and ED releases
- Works closely with medical control in all patient care utilizing specified and approved protocols
- Closes the gaps of communication between EMS agencies, physicians, and other health care
providers
- Completes and submits all documentation/paperwork requirements daily.
- Monitors remote patient tracking and responds as appropriate in a timely manner.
- Interacts with EMS and ER personnel as necessary, and with expediency, to coordinate emergency
care for any patients presenting with risk factors that warrant an immediate trip to the hospital
- Completes an equipment checklist at the beginning of each assigned shift. Inspects and cleans the
assigned vehicle and all equipment. Ensures compliance with minimum stock requirements, proper
function and cleanliness of all equipment.
- Ensures deployed devices are tracked and in good working order at all times.
- Provides appropriate documentation of patient intervention as per company standard
operating procedures.
- Attends Interdisciplinary Team (IDT) meetings and provides additional information on patients and
families they interacted with
- Additionally, assists with:
- Patient Engagement
- PCP Engagement
- Tuck-in Calls
- Help the rest of the team, as the physician sees fit (e.g., home visit for safety check,
non- clinical urgent matters, etc.)
- Attend all required trainings, including equipment/device training. Ongoing training to be
expected and additional education, skills, and protocols will be added.
- Operate the company vehicle in accordance with safety rules and regulations; company policies
and procedures; federal, state, and local laws, regulations, and statutes
- Maintain all required certifications as an active Licensed Practical Nurse and supply valid copies of
all certifications and driver’s license prior to their expiration dates as well as submit a copy of
driver’s license for the yearly MVR pull.
- Other tasks needed to accomplish team’s objectives/goals.
- Operate and drive the company vehicle in a safe and professional manner while exercising due
regard for self and other motorists at all times
- Ability to have compassion for patients in all interactionsQUALIFICATIONS
Educational/ Experience Requirements:
- LPN Certification
- Minimum of three (3) years as a LPN in a direct patient care role
- Possess a valid driver’s license with an acceptable driving record
- Must be in good Clinical standing pass the scenarios test and basic Physical abilities test
- Advanced Medical Life Support (AMLS), CT, CCP are preferred. State required PALS,
BLS, ACLS cards must be current.

Job Types: Full-time, Travel nursing

Pay: $50,000.00 - $74,000.00 per year
