Medical Assistant/Patient Care Coordinator
Job description
Seeking an experienced Medical Assistant/Patient Care Coordinator to join our fast-paced and growing company.
You will be expected to provide outstanding customer service to patients by working independently as well as collaboratively within a team environment. Understand high-risk medical conditions, medications procedures, basic wellness screening requirements and HEDIS measures. You will function as an educator, healthcare resource, consultant, and patient advocate by working in an office setting. This is a full-time position with salary and full benefits.
- Certified Medical Assistant (Required)
- EMR systems: 1 year (Preferred)
Medical Assistant/Patient Care Coordinator Duties and Responsibilities:
- Effectively communicate with patients by managing inbound/outbound calls and emails throughout out the workday. Educate our patients on their health conditions and the importance of following their health care needs.
- Assist in varieties of need a patient/physician may need to better care for the patient. Communicate with physicians(s) offices or case managers. Manage patient screening, scheduling visits.
- Manage by following the patient under hospice. If patient expires update physician office and chart.
- Coordinate and update patient charts through electronic medical records
- Accurately and timely update records to ensure all parties have accurate information
- Coordinate multiple responsibilities across the care of the patient(s)
- Verify TCM of follow-up appointments are being made for the patient
- Medication/HEDIS measures are completed
- Update Metric reports
- Assist with the identification and reporting of potential quality management issues-Other duties as assigned from time-to-time
Medical Assistant Qualifications: * Must have at least 2 years’ experience working as a medical assistant.
- Must have proficient computer skills and be comfortable with EMR Athena preferred
- Must possess a positive attitude, medical knowledge, be energetic and demonstrate both excellent customer service and polished communication skills.
- Must also be detail-oriented, exceptionally organized, able to follow directions, accept correction, multi-task, and work as part of a team.
- Knowledge of medications is a plus. Knowledge of MRA coding, HEDIS, Wellness Visits and Meaningful Use is a plus. Bilingual (Spanish) also a plus.
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
