Bachelor's degree in Social Work, Sociology, Psychology, Nursing or related field and three years of related work experience or an equivalent combination of education and experience.

Position Description:

3 available positions

Assist HIV infected individuals to access the Ryan White Part B services through enrollment into the program.��

Essential Duties and Responsibilities
  • Provide initial assessment of client medication regimen and treatment plan and medical and non-medical service needs; develop a comprehensive individualized care plan; document the coordination of services to implement care plan and monitor the plan to assess efficacy.
  • Provide benefit / entitlement counseling and referral services in a timely manner for medical and non-medical needs by assisting clients to access public and private service programs for which they may be eligible.
  • Provide follow-up care coordination for HIV+ patients that have been out of adherence longer than six months.
  • Provide clients with educational information regarding disease transmission and prevention methods and encourage a healthy lifestyle over the course of continued case management.
  • Participate in ongoing HIV education and attend Ryan White Part B services conference and seminars.
  • Document all client interaction and services provided in the electronic medical record and CareWare database.
  • Participate in clinical quality management activities as assigned.
  • Identify and document patterns of client service needs.
  • Other duties as assigned.
