Assist HIV infected individuals to access the Ryan White Part B services through enrollment into the program.��
Essential Duties and Responsibilities
- Provide initial assessment of client medication regimen and treatment plan and medical and non-medical service needs; develop a comprehensive individualized care plan; document the coordination of services to implement care plan and monitor the plan to assess efficacy.
- Provide benefit / entitlement counseling and referral services in a timely manner for medical and non-medical needs by assisting clients to access public and private service programs for which they may be eligible.
- Provide follow-up care coordination for HIV+ patients that have been out of adherence longer than six months.
- Provide clients with educational information regarding disease transmission and prevention methods and encourage a healthy lifestyle over the course of continued case management.
- Participate in ongoing HIV education and attend Ryan White Part B services conference and seminars.
- Document all client interaction and services provided in the electronic medical record and CareWare database.
- Participate in clinical quality management activities as assigned.
- Identify and document patterns of client service needs.
- Other duties as assigned.
