As the Memory Care Coordinator at Belmont Village Cardiff by the Sea, you will directly manage and support all dementia care staff, dementia activity professionals, complete resident cognitive assessments and work collaboratively with a team to develop plans of care. You will manage the overall operation of the dementia care department in accordance with Belmont Village standards and state regulations. This position is a hands on management role including a high level of engagement with employees, residents and families.

ESSENTIAL RESPONSIBILITIES

  • Management of all dementia caregivers and dementia activity leaders
  • Conducts advanced Alzheimer's / dementia training for all staff & is the community dementia expert.
  • Conducts cognitive assessments on prospective & current residents translating outcomes to service plans related to care & programming.
  • Works collaboratively with community management, regional & corporate support teams.
  • Effectively communicates with, educates & offers support to resident family members as needed.

QUALIFICATIONS

  • Must be fully vaccinated for COVID-19 and the flu for employment in California
  • Bachelors or Masters degree in Social Work, Therapeutic Recreation, Gerontology or related field.
  • Three (3) years hands-on dementia / Alzheimer's activity program facilitation experience
  • Minimum 2 years management experience in a comparable healthcare setting
  • Flexibility to work and support a 24/7 community operation
  • Demonstrated successful use of technology in the workplace
