JOB OPENING: MENTAL HEALTH INTAKE COORDINATOR
LOCATION: West Salem Clinic – 1233 Edgewater St. NW
HOURS: Monday - Friday, 8:00am - 5:00pm

ABOUT OUR MENTAL HEALTH CLINIC:
West Salem Mental Health Clinic offers a full range of mental health services for patients enrolled at our West Salem and Total Health Community Clinics. Our Medical and Mental Health care teams are committed to the overall wellness of our patients and work together to ensure patient needs are expertly evaluated and met with compassion and respect. We employ highly qualified, well-trained medical and mental health providers including psychiatrists, psychiatric nurse practitioners, licensed clinical social workers, counselors, case managers, and administrative personnel.

YOUR ROLE:
Facilitate access to mental health services for patients of the West Salem Medical Clinic and Total Health Community Clinic. Works as part of the care team to perform preliminary screenings, complete initial assessments, formulate treatment plans, and assign therapists.

CORE COMPETENCIES:

  • Coordinates all intake services and mental health integrated delivery system (IDS) screenings in accordance with County and agency standards for Northwest Human Services. Establishes rapport, analyzes, and interprets all relevant data to determine treatment recommendations and priorities. When care is best provided outside of NWHS the MH Intake Coordinator facilitates that handoff by contacting the receiving agency and coordinating transfer of care until client has engaged in services or expresses that they no longer want care.
  • Serves as back up for Behavioral Health Consultants in Primary Care and when there is a no show or cancellation, will assist therapists with case closures.
  • Works with Quality Management, Clinic Manager, and the Clinical Supervisor to maintain and update the waitlist for intake, the Mental Health Intake Packet to ensure all relevant and required forms are included and up to date. May bridge clients on waitlist when clinically appropriate.
  • Monitors regulatory body requirements and ensures access to care is obtained within expectations by effectively managing time, executing job duties, and being available for warm handoffs from the care team and providing warm handoffs to outside agencies when necessary. Alert’s clinic leadership when access is outside agency expectations.
  • Screens, assesses, coordinates services as clinically indicated and develops a plan for treatment that addresses both mental health concerns and co-occurring disorders, while ensuring complete and timely documentation pursuant to OAR 309.019.0140.

QUALIFICATIONS:

  • Master’s degree in a mental health discipline and be actively working towards licensure, be eligible for licensure in the State of Oregon or be licensed as a LPC, LMFT, or LCSW in the state of Oregon.
  • Graduate course work in theory, diagnosis, and treatment of mental health disorders.
  • Proficiency using DSM IV-TR, DSM V, and ICD-10.
  • Experience working with persons who have mental health issues and those who are in emotional distress.

Additional Preferred Experience & Skills:

  • experience in a medical clinic are preferred.
  • Experience in an integrated system.
  • NexGen experience.
  • Bi-lingual Spanish speaking candidates highly preferred.
