Job description
We are looking for a Social Worker to provide individual therapy to individuals with persistent mental illness, in the community. The Therapist will be a member of a treatment team that provides therapy and case management services to individuals. General responsibilities include :Serves as the primary therapist and treatment coordinator for a mixed caseload of persistently mentally ill patients
Provides individual, group, and/or family therapy as needed
Provides in-home services to individuals on the mobile service
Under the supervision of a psychiatrist, assists in the monitoring of the prescribed use of psychotropic medication
Treatment planning
Provides information to patients and their families concerning the appropriate use of the Center’s services and available community resources
Provides information to community residents, groups and organizations regarding services and programs available at the Center
Provides case management services as needed to their patients
