No Wrong Door (NWD) Independent Living Specialist

Job Summary: No Wrong Door Specialist will provide information, referral, assistance, and advocacy to individuals, family members, and their caregivers seeking Long Term Services and Supports (LTSS). Work in conjunction with the designated NY Connects agencies to fulfill No Wrong Door service requirements including providing direct services to individuals, family members, and their caregivers; participate in monthly No Wrong Door meetings; and conduct community outreach with partnering agencies on accessing LTSS.
Job Duties:
- Provide barrier-free information and assistance (including home visits if necessary) to enable clients access to community LTSS services.
- Perform information and assistance functions using the NWD PeerPlace data system for NY Connects including phone calls, walk-ins, and off-site visits. Collect prescribed information on individual, family member, and caregiver needs including financial and program eligibility as well as types of services and resources that may best meet their needs.
- Follow NY Connects NWD protocols to provide application assistance including helping clients complete applications, submitting completed applications, determining eligibility for programs, and providing linkage and referral to other partners for necessary supports.
- Work with the local NY Connects program agencies to conduct outreach and education about enhanced LTSS including Consumer Directed Personal Assistance Services (CDPAS), Personal Care Services (PCS), Durable Medical Equipment (DME), Waiver Services, Healthcare Advocacy, Benefits Advisement, and other enhanced LTSS.
- Maintain regular presence at designated county NY Connects agencies as part of RCIL's (Regional Center for Independent Living) four-county service area (Monroe, Wayne, Ontario and Yates County).
- Facilitate safe transitions for individuals transitioning from institutional to home and community-based settings.
- Work with NY Connects NWD to implement the State evaluation plan for consumer, program, and systems.
- Participate in local monthly NWD meetings and Long Term Care Council meetings.
- Submit documentation in a timely manner and maintain organized documentation and case notes in online PeerPlace and CSR database and file documents in an efficient manner.
- Submit monthly reports in a timely manner to IL Manager for NY Connects monthly tracking.
- Carry out other assignments as needed and requested by the IL Manager and Director of IL.
Qualifications:
- High school diploma required. Associate degree preferred in addition to two years of experience in the Human Services field.
- Knowledge and experience accessing and advocating for LTSS such as CDPAS, PCS, DME, and Waiver services, through Medicaid and other community-based services.
- Knowledge and experience with benefits programs including the appeal processes for Medicaid, Temporary Assistance, Supplemental Nutrition Assistance Program, Supplemental Security Income, and Social Security Disability Insurance.
- First hand experience with a range of disabilities is valuable as well as being able to share that knowledge of disabilities with others effectively.
- Individuals with disabilities with personal life experience navigating LTSS are encouraged to apply.
