About us

Working as a nurse home visitor is much different than hospital or clinical nursing. Our nurses get to work one on one with families during their pregnancy and then for two years after they have their babies. This allows the nurses to support the client during this critical time in their and their babies’ lives. It also allows nurses to really spend quality time with their families.

Instead of having six to twenty patients in your care a day each nurse sees about twelve clients a week. This means that they see about three families a day. Instead of rushing from patient to patient and dealing with the seemingly endless charting, our nurses spend an hour to an hour and a half with each family and have some documentation to complete related to that visit. Our nurses also receive intensive training and support. Each nurse receives two months of training prior to seeing families independently, attends weekly meetings with all of the nursing staff, and has time one on one with the nursing supervisor to debrief on their week.

Nurse Visitor
Family Visitor Programs was established to address community concerns about increases in child abuse identified by a needs assessment in the 1980s. The Home Visitor Program began providing services to families in the fall of 1983, as one of fifteen demonstration projects with seed money from a federal Maternal Child Health block grant. In November 1986, the program became an independent not-for-profit corporation. Because of community support, the agency was the only “demonstration” project to survive, and is the longest established new parent home visitation program in Colorado.

The agency provides home visitation services to pregnant women and to parents of young children residing in the Aspen to Parachute, Colorado region. Our programs are offered to families at no cost, regardless of income.

Nurse Family Partnership Visitor
Overview : The Family Visitor Nurse-Family Partnership (NFP) program provides case management and education through home visitation for low-income, first-time pregnant women and their families from Aspen to Parachute Colorado. Nurse home visitors conduct home visits from the early prenatal period to the child's second birthday.

Qualifications and Skills

Applicants must be a Registered Nurse and have a current Colorado nursing license, CPR certification and have knowledge of nursing practices and experience working with pregnant women, infants and toddlers.

A valid State of Colorado driver's license and use of a personal, insured vehicle will be required to perform job duties. Auto insurance, maintained at all times, as required by state law.

Bilingual (Spanish speaking) preferred.

Part time position is Monday thru Saturday 20 hours/week. Paid Holidays (10), Vacation, and Sick Time.

  • Establish and maintain therapeutic relationships while delivering individualized client case management services.
  • Conduct home visits to determine patient and family needs. Assess the physical, emotional, social, and environmental needs of clients and their families as they relate to health and life-course development and develop plans to meet these needs according to program guidelines for a caseload of 25+ clients. Evaluate progress of family members toward targeted goals and revise plan of care, as appropriate.
  • Provide support to assist clients and their families in attaining targeted goals in areas that include prenatal care, nutrition, parenting, well-child care, family planning, and special healthcare needs.
  • Teach clients about healthy behaviors, provide technical expertise to clients regarding best practices for appropriate provision and care for their child's/children's and family's health and well-being. Assist clients and their families in establishing realistic goals and measurable outcomes for promoting their health and life-course development.
  • Develop an effective working relationship with clients and their families to enhance problem-solving competence. Use logic and reasoning skills to identify the strengths and weaknesses of each family and guide them to consider alternative solutions or approaches to problems when appropriate. Consider the relative costs and benefits of potential actions to help the family choose the most appropriate solution for them. Promote the involvement of other client family members during pregnancy, birth, and early childhood.
  • Provide education, resources, and referrals as appropriate. Identify gaps and barriers in community systems for clients and families. Consult, coordinate and collaborate closely with community resources to link families with appropriate services. Refer clients with social and emotional problems to other community agencies for assistance. Serve as a resource to other community agencies.
  • Responsible for the documentation for assigned clients and maintain records of patient care, condition, progress, or problems to report and discuss with supervisor.
  • Work independently while maintaining strong professional boundaries.
  • Maintain current nursing license.
  • Maintain sensitive and confidential records information.
  • Drive for extended distances and periods of time from Aspen to Parachute in all weather conditions throughout the year.
  • Complies with federal, state, and local regulations and employer policies and procedures.
