Position Summary:

Assumes responsibility and accountability for optimization of the patient flow by coordinating resources with healthcare teams to maximize service quality and an efficient mentioned. Responsible for following patients.

Responsibilities

Coordinates care for patients and families. Provides initial patient contact. Develops plan to assist patients with scheduling. Identifies patient trends, receives referrals from patients, coordinates housing for patients.

Qualifications:

Education

Required: Masters degree in Nursing.

Preferred: Master's degree in Informatics or related health care field.

Experience

Required: 1 year of experience.

Certifications

Required: NP license

$72.00 - $115.21

Wages are determined based upon a number of factors including, but not limited to, an employee�s qualifications, experience and personal productivity.

Pay Range: $59.24 - $94.79
