Advantage is currently seeking a Full-Time Nurse Practitioner to join our vastly growing Palliative Team in the Mechanicsburg, Pa area! This position is responsible for providing patient care in a continuing care retirement community.
The Palliative Care Nurse Practitioner functions as an independent clinician in assessment, planning, treatment, communication and identification of goals of care for patients with serious illness. The Palliative Care Nurse Practitioner collaborates successfully as a member of the multidisciplinary Palliative Care Team, taking responsibility for quality and continuity of care of assigned patients.

Responsibilities:
- The Nurse Practitioner provides education and leadership in the principles of palliative care for the primary nursing and medical teams that interface with the Palliative Care Team.
- The Nurse Practitioner embodies and serves as an ambassador for the philosophy of evidence-based symptom management and whole patient assessment for patient with serious illnesses referred to the team.
- In this role, the Nurse Practitioner moves the practice forward in evidence-based management of symptoms.
- The Nurse Practitioner shows interest in and willingness to participate in research with the team.
- The Nurse Practitioner works with the team to facilitate clarification of patient and family goals of care and access to appropriate support models of care i.e., hospice, rehabilitation, on-going aggressive treatment.
- Educates staff and the community on all aspects of palliative care and the services provided. Attends and participates in team meetings.
- Prepares and maintains accurate patient records, charts and documents to support the medical practice and reimbursement for services provided.
- Prescribes medications including controlled substances to the extent delegated and licensed and after collaboration with palliative care and referring physician.
- Orders diagnostics/treatments as needed after collaboration with palliative care and referring physicians.
- The Nurse Practitioner will consult with supervising palliative care physician, when clinically appropriate, for the purpose of executing physician orders.
- Assists in the development of clinical practice guidelines/standards in support of quality palliative care with the team.
Qualifications:
- Master's of Science in Nursing- MSN
- Candidate must be a graduate of an accredited PA State Nursing Board approved Nurse Practitioner Program.
- Licensed to practice in the state of PA; certified as a Nurse Practitioner.
- Board Certified or Board Eligible in Hospice & Palliative Medicine
- Minimum of 2 years’ experience preferred
- Certification in Generalist Hospice and Palliative Nursing preferred (CHPN)
- Advanced Certification in Hospice and Palliative Nursing preferred (ACHPN)
