Nurse Practitioner / Physician Assistant
Job description
Resident Care:
- Provides routine health care to residents as assigned.
- Provides urgent care and follow-up care of problems as required for all residents and Day Service registrants.
- Provides emergency care of employees, volunteers and visitors until definitive care through own physician or
- Participates in interdisciplinary care planning as needed.
- Assists physician as appropriate in admission and re-admission medical evaluations.
- Provides counsel, support and education to residents and their families in conjunction with the health care team.
- Is available for evening, weekend and holiday clinical care on a shared on-call basis.
- Works closely with all members of the health care team to ensure high quality medical care for all residents.
- Performs employee physical exams for new employees.
- Completes and submits resident visit Encounter Sheets in a timely fashion (within 24 hrs)
- Completes documentation of patient/resident visits in EHR within 24 hrs
- Assists in development of and compliance with health, medical and quality improvement procedures of the
- Assists in evaluation and implementation of clinical training, education and/or operational systems of the Home.
Resident Care:
- Maintains appropriate communication with involved physicians and documents activities
Staff:
- Participates in in-service and other educational programs to improve the skills and knowledge of other staff
- Works closely with Infection Control staff
- Assists in development and maintenance of an employee health service, involving injury/illness assessment,

Administrative/Additional Responsibilities:
- Serves on various committees as appropriate.
- Assists in medical research, scholarly work, or special projects as appropriate.
- Acts in accordance with all required State and Federal regulations.
- Maintains licensure to practice medicine in New York State.
- Supports and ensures residents’ rights.
- Other duties as assigned.

RELATION TO OTHERS: Daily contact with and ability to interact courteously and tactfully with staff, residents,
family members, Day Service participants and visitors.
ADVERSE WORKING CONDITIONS: May be exposed to infections, disease and disagreeable odors.
PHYSICAL AND MENTAL DEMANDS: Sits 50-75% of the day. Must be able to lift 10-30 pounds (most frequently
10-15 pounds). Occasional walking, stooping, squatting and bending. Repetitive hand/wrist movement. Clinical problem
solving, subject to frequent interruptions. Sensitive to the needs/problems of the geriatric resident. Physical Intensity
Level 2.

WORK ENVIRONMENT: Well lighted, ventilated office as well as throughout the facility.
VARIABLES: May be required to work at times other than normal scheduled hours.
