JOB SUMMARY

The Nurse Specialist is responsible for facilitating coordination of care services for pediatric patients in need of these services. Care coordination will include taking calls from patients via telephone and/or in-person evaluation and providing patients with education, counseling, and assistance in the resolution of issues regarding their diagnosis; care management; medications, including medication refills per protocol; appointments; triage of acute conditions.

EDUCATION and/or EXPERIENCE

• Registered Nurse and Nursing Diploma with three years of experience as a nurse or other health related position OR
• Associate’s Degree in Nursing (ADN) with five years of experience as a nurse or other health related position OR
• Bachelor’s Degree in Nursing (BSN) with one year of experience as Nurse or other health related position OR
• Master’s Degree in Nursing or other related field plus one year of nursing experience.
• Other acceptable relevant experience may include: other health related position, working with patients with chronic conditions, care coordination, and care management in a medical setting, and managing competing priorities.

Job Type: Full-time

Pay: $30.00 per hour
