Nursing Immunization Coordinator - LVN (Atwater)
Job description
Responsible for assessing, planning, monitoring and implementing vaccine management activities. Work collaboratively with internal and external stakeholders to promote and improve immunization rates and practices.
Schedule is Monday – Friday, 8:00am – 5:00pm
Essential Duties and Responsibilities
- Integrates the organization’s established best practices and health promotion campaigns into immunization practices organization-wide.
- Reconciles vaccines received with orders placed and assures adequate rotation of existing vaccine to prevent loss.
- Manages and accounts for Vaccines for Children (VFC) inventory organization-wide.
- Develops and maintains immunization inventory/usage tracking systems.
- Promotes and facilitates use of immunization registry within GVHC.
- Develops strategies and processes to improve the immunization status of children.
- Works in collaboration with appropriate departments and/or personnel to provide immunization outreach clinics for improvement of immunization compliance and establishment of care.
- Trains staff and assesses competency for vaccine administration, storage/handling, and quality assurance (QA) protocols.
- Resource for patient and/or staff questions and concerns regarding immunizations.
- Monitors the performance of the immunization program by conducting chart audits and physical site surveys to ensure compliance with QA policies and procedures.
- Assures adherence to the use approved branded vaccines at the site level as directed by the Clinical Committee.
- Conducts trainings and audits to ensure immunizations are properly documented within the electronic medical records system.
- Acts as a liaison with county agencies and VFC for vaccine related matters, participating in pilot projects as directed.
- Mitigates vaccine loss and performs risk management activities related to loss and recovery of vaccines, equipment, and patient events related to immunization administration.
- Gathers data and reports on vaccine related activities.
- Additional duties and/or responsibilities as assigned.
Physical Demands
- Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
- Ability to exert maximum muscle force to lift, push, pull or carry patients.
- Must be able to hear staff/clients on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
- Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
- Must have high manual dexterity.
- Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
Work Environment
- Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
- Subject to unpleasant odors.
- The noise level is usually quiet to moderate, but may at times be noisy and crowded.
Education/Experience Requirements
Minimum Qualifications:
- Valid CA Driver’s License, acceptable driving record, and vehicle insurance.
- Ability to relate in a professional manner and handle confidential information.
- Ability to function in a team environment.
- Organized and detail oriented, with specific ability to maintain accurate records.
- Excellent verbal and written communication skills.
- Excellent customer service skills.
- Must be able to handle multiple priorities to meet established deadlines.
- Proficient in Microsoft Office applications.
- Ability to collect, organize, manage and report on large volumes of meaningful data for decision making while using spreadsheets or other data processing software.
- Must be able to interact with all levels of GVHC employees.
- Must possess the ability to work independently, exercise initiative and good judgment, solve practical problems and deal with a variety of variables.
- Bilingual English-Spanish, preferred.
Education/Experience
- High school diploma or equivalent.
- Graduate of an accredited school of professional nursing.
- Current unencumbered Licensed Vocational Nurse (LVN) in the state of CA.
- Current BLS CPR Card certified by American Heart Association.
- Minimum one (1) year of general pediatric and immunization experience.
- Supervisory experience, preferred.
