Applying the principles of Systems of Care, the Supervisor of OhioRISE Care Coordination is responsible for developing and overseeing a team of Care Coordinators who ensure and cultivate flexible, family-focused, community-based responsive services based on the High-Fidelity Wrap Around Model of care coordination for Tier II Moderate clients and Tier III Intensive clients in alignment with The Wingspan Care Groups Mission, Vision and Core Values.
Essential Tasks, Duties, and Responsibilities:
Participate in the recruitment, interviewing and training of potential new Care Coordinators.
Ensure achievement of monthly and yearly goals for staff and program.
Provide both formal and informal training for assigned staff; training should be individualized, specialized and ongoing.
Provide monthly utilization review for the team.
Conduct performance evaluations and develop action plans for assigned staff for continued professional development. Develop corrective action plans as needed.
Provide regular supervision with assigned staff to ensure open lines of communication - provide both verbal and written feedback.
Facilitate meetings with staff, at least on a bi-weekly basis or as defined by regulatory expectations, and other departmental meetings to ensure quality, holistic treatment of families and consistency in program planning.
Reviews program specific paperwork to ensure ongoing quality with documentation and around treatment planning.
Ensures adequate staff coverage and crisis coverage for clients in cases of staff absences.
Ensures development of an appropriate aftercare plan regarding ongoing client needs.
Oversee the development and implementation of all treatment and service plans for clients and their families.
Ensure program supervision is provided to staff who are making decisions about the most appropriate service for a client and family.
Ensure that staff is trained in the developmental/treatment strategies outlined in the treatment plans.
Ensure that staff is utilizing approved program strategies when working with clients and their families.
Participate in required fidelity reviews as coordinated by Case Western’s Child and Adolescent Behavioral Health Center of Excellence.
Complete written work that is legible, accurate, uses professional language, and meets established timelines.
Other duties as assigned.
Knowledge, Skills, and Abilities:
May be licensed or an unlicensed practitioner in accordance with rule 5160-27-01 of the Administrative Code.
Must complete the high-fidelity wraparound training program provided by an independent validation entity recognized by ODM.
Must successfully complete skill and competency-based training to provide Moderate Care Coordination (MCC).
Bachelor’s degree with 2 years’ experience and license, or Master’s degree or higher with 1 year of experience, in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth, and their family or caregivers.
Have a background and experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
Be culturally competent or responsive with training and experience necessary to manage complex cases.
Have the qualifications and experience needed to work with children and families who are experiencing SED, trauma, co-occurring behavioral health disorders and who are engaged with one or more child-serving systems (e.g., child welfare, juvenile justice, education).
Ability to use a computer; proficiency in Word, Excel and Electronic Health Record (EHR).
Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing Wingspan Care Group with vehicle insurance.
Ability to visit clients’ homes (may or may not be handicapped accessible).
Excellent verbal and written communication skills; strong teamwork and organization/time management skills.
Physical Demands:
Occasionally move about inside the office to access file cabinets, office machinery, etc.
Constantly operates a computer and other office productivity machinery, such as a copy machine, computer printer, etc.
Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
Ability to handle bending, stooping, lifting, pushing, reaching, and walking for periods of time.
Must be able to lift 20 pounds independently
