Schedule: 8 am-5 pm (10 hr. days available) position could be filled as full-time, or part-time 2-3 days a week, depending on candidate and agency need.
Overview
This positon works with the Behavioral Health Initiative for Older Adults and People with Physical Disabilities (Behavioral Health Initiative). The goal of the position is to improve the timely access to care from qualified providers who work together to provide coordinated, quality and culturally responsive behavioral health and wellness services to this population. This position works extensively with community partners that serve the older adult and people with disabilities population. This position coordinates complex case consultations on behalf of older adults for the Josephine County community. Provides community education on the special needs of older adults. The person in the role needs to have a strong interest and experience in working with older adults and individuals with disabilities.
Responsibilities
Works independently in the community and maintains professional relationships at all times.
Attends all mandatory meetings, participates in regular individual and group supervision. Maintains positive working relationships. Works as an active and cooperative member of treatment team.
Works collaboratively with community partner organizations such as Aging and People with Disabilities and the Rogue Valley Council of Governments on the needs of older adults and individuals with special needs.
Compiles and completes quarterly reports to OHA on activities.
Works to develop a semi-annual plan to address the goals of the initiative.
Works to identify individuals in the mental health system that are turning 60 + to ensure their needs are met.
Coordinates and staffs cases with community partners to find ways to remove barriers for clients needing care.
Works with other Older Adult Specialists across the state to provide community education on the special needs of the aforementioned population.
Coordinates care with medical providers, local foster care providers, CRC, senior and disabled services, and other service providers.
Facilitates referrals to appropriate internal and/or external resources where indicated.
Provides case consultation and education services within the agency and with community partners as needed.
Qualifications
Master’s degree in counseling, social work, or a related behavioral health field, or Bachelor’s/Master’s degree in nursing, from an accredited college or university.
Must be a QMHP on the Mental Health & Addictions Certification Board of Oregon (MHACBO) registry.

OR
Must be registered as a licensure intern with the State of Oregon or be licensed in Oregon as an LMFT, LCSW, LPC or clinical Psychologist.
Ability to meet people with ease; ability to write and edit effectively; ability to communicate the written and spoken word with tact, diplomacy, and/or authority when necessary; good organizational skills, well disciplined, able to meet deadlines, self starter, effective public speaker. Thorough understanding of evidence-based supported employment.
Must pass state-required background and DMV checks. Candidate must be able to work independently and flexibly, under general supervision.
