This is a part-time position, up to 29 hours per week. Days and hours of work are Monday through Thursday 8:00 am to 5:00 pm and Friday 8:00 am to 2:30 pm, with 30 minute lunch breaks. Occasional evening and weekend work may be required as job duties demand. This position is located in Lowndes County, GA. Part-time positions are not benefit eligible.

Under the supervision of the HIV Prevention Coordinator and the Healthcare Program Consultant, the Outreach Services Worker/Pre-exposure Prophylaxis (PrEP) Navigator will provide outreach to relevant communities through HIV testing and counseling to offer education and to identify HIV positive individuals. The Outreach Services Worker/PrEP Navigator will also provide support and services to HIV negative individuals who require assistance in accessing PrEP care. This individual will work with the Lowndes County Health Department staff, other health departments within the district, and the Infectious Disease (ID) HIV Prevention Team to identify potential PrEP clients and those in need of PrEP navigation services.
Job Responsibilities:
- Provides in-house and community outreach to high-risk populations to support HIV prevention
- Provides rapid HIV testing and risk assessment counseling to clients
- Provides educational support for individuals regarding HIV risk reduction and PrEP services
- Works collaboratively with district health department staff, ID clinic staff, and community partners
- Makes appropriate referrals for clients to the Linkage Coordinator
- Assessment of clients' level of risk to determine appropriateness for PrEP
- Assesses and addresses client's knowledge, attitudes, and beliefs regarding PrEP as well as their intentions to initiate and comply with a PrEP regimen
High School Diploma or GED

The use of wellness breaks during the workday is authorized to support this philosophy and assist employees in meeting their wellness goals. A maximum of 30 minutes in a given workday may be used to engage in wellness activities, generally in the form of two 15-minute breaks or one 30-minute break.
