Job description
This position works with eligible families in providing education regarding child development, parenting, family health and wellness, and performing developmental screenings using the Parents as Teachers curriculum. This position also links program participants and other clinic patients to community resources as well as provides case management interventions to address unmet needs and achieve optimal outcomes.
Parent Educator range: $21-$25.25 DOE with ability to go higher for highly experienced candidates.
Responsibilities:- Works as part of team to manage the needs of eligible clients to improve access to care and improve outcomes
- Case Management
- Perform home visits and travel to remote clinic sites to support program activities and other work related activities and/or trainings
- Other duties as assigned.
- Promotes a positive work culture and upholds clinic policies and procedures, including the WE CARE Standards of Behavior.
- Must uphold confidentiality at all times in regards to CBHA activities, reports, financials, patient health information and other proprietary information specific to CBHA.
Professional and Technical Knowledge:
- Job duties require specific knowledge and training in early childhood development, case management and patient education, typically acquired during completion of an on-the-job training program with duration of up to a year.
Licenses and Certification:
Possesses a valid driver’s license and meet company insurance liability standards as well as the ability to travel to perform home visits and other work related activities.
Technical Skills:
- Ability to prepare basic correspondence and simple reports in Microsoft Word.
- Ability to use Microsoft Excel to create tables and simple displays of information.
- Ability to create basic presentations in Microsoft PowerPoint.
- Ability to perform basic e-mail functions using Outlook.
- Ability to enter and manage data into a centralized electronic health records.

403 (b) Retirement Plan:
Safe Harbor Match: Employees are eligible to receive a Safe Harbor Match of 100% up to 4% of the employee’s total salary. Employee is full vested in Safe Harbor Match from day 1. Contributions are subject to IRS guidelines.
Non-Elective Contribution- Provided the employee has met enrollment qualifications, employee will also receive 4% employer contribution of employee’s total salary at the end of the Plan Year at the discretion of Board of Directors.
