Shift: Days, 8am – 4:30p

• Located on a lush tropical campus, our flagship hospital, 1,368-bed AdventHealth Orlando serves as the major tertiary facility for much of the Southeast, the Caribbean and South America
• AdventHealth Orlando houses one of the largest Emergency Departments and largest cardiac catheterization labs in the country

The role you’ll contribute:

The Patient Experience Facilitator is responsible for supporting and promoting patient and family centered care for the AdventHealth team. The facilitator works closely with patient experience and unit leadership to support and foster effective communication between families and clinical team. The facilitator would assist in ensuring quality multidisciplinary rounds across various Pediatric and Obstetric units. This includes but is not limited to bi-directional communication with patients and families, virtual rounding and ensuring mutual understanding after multidisciplinary rounds. The facilitator will assist in increasing communication and promoting safe, quality care. This may include supporting virtual connections, maintaining the Family Resource center, and discharge phone calls. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

• Collaborates with unit leadership, team and patient experience leadership to ensure improvements in patient and family centered care.

• Perform discharge phone calls for pediatric patient’s post-transition home.

• Provides guidance and coaching for team members during rounds to support effective family centered communication.

• Ensures successful bi-directional communication and coordinates further understanding.

• Facilitate follow-up conversations with patient caregivers when further follow up is needed.

• Assists with connecting families virtually during rounds if they are unable to attend in person
