Good Shepherd Rehabilitation Network has an immediate need for a per diem Pharmacist to work at our Rehabilitation Hospital in Allentown, PA.

JOB SUMMARY:

Serves patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies; recommending alternative medications when appropriate.

HOURS:

  • Variable, as needed
  • Shifts available between 8:00 AM – 8:00 PM
  • Must have weekday availability

ESSENTIAL FUNCTIONS:

  • PREPARES MEDICATIONS
  • By reviewing and interpreting physician orders
  • reviewing patient clinical information
  • verifying that the correct dose has been ordered based on age, weight and laboratory information
  • reconciling medications against the patient’s list of home meds
  • and detecting therapeutic incompatibilities.
  • DISPENSES MEDICATIONS
  • By compounding, packaging, and labeling pharmaceuticals.
  • Utilizes Pyxis technology to deliver medications to patients with quicker turn around time and increased safety.
  • CONTROLS MEDICATIONS
  • By monitoring drug therapies
  • complying with therapeutic interchanges, and advising interventions.
  • Records all medication interventions with prescribers, provides information to doctors, and follows up to ensure that intervention advice has been reviewed by the physician.
  • COMPLETES PHARMACY OPERATIONAL REQUIREMENTS
  • By organizing and directing technicians' work flow
  • verifying their preparation and labeling of pharmaceuticals
  • verifying order entries, charges, inspections and other related activities.
  • PROVIDES PHARMACOLOGICAL INFORMATION
  • By answering questions and requests of health care professionals
  • counseling patients on drug therapies, and by advising the prescriber if the pharmacist identifies any irregularities or safety concerns that need to be addressed by the responsible physician.
  • Utilizes technology to access clinical data in order to make better clinical recommendations.
  • DEVELOPS HOSPITAL STAFF'S PHARMACOLOGICAL KNOWLEDGE
  • By participating in clinical programs
  • training pharmacy staff, students, interns, externs, residents, and health care professionals.
  • COMPLIES WITH STATE AND FEDERAL DRUG LAWS AS REGULATED BY THE STATE BOARD OF PHARMACY, THE DRUG ENFORCEMENT ADMINISTRATION, THE FOOD AND DRUG ADMINISTRATION AND ALL OTHER APPROPRIATE AUTHORIZED REGULATORY AGENCIES/BODIES
  • By monitoring nursing unit inspections
  • maintaining records for controlled substances
  • removing outdated and damaged drugs from the pharmacy inventory
  • supervising the work results of support personnel
  • maintaining current registration
  • studying existing and new legislation
  • anticipating legislation
  • advising management on needed actions.
  • PROTECTS PATIENTS AND TECHNICIANS
  • By adhering to infection-control protocols and by ensuring that every product dispensed is for the right patient, at the right dose, and delivered in a timely manner.
  • MAINTAINS SAFE AND CLEAN WORKING ENVIRONMENT
  • By complying with procedures, rules, and regulations.
  • CONTRIBUTES TO TEAM EFFORT
  • By accomplishing related results as needed.
  • DEMONSTRATES SKILL AND EFFECTIVENESS IN ASSESSING PATIENT/CLIENT NEEDS AND IN PROVIDING COMPETENT CARE AND SUPPORT IN AGES INFANCY THROUGH GERIATRIC
  • By reviewing hospital electronic medical records and interacting with appropriate individuals as needed.
  • ENSURES SECURITY OF MEDICATIONS
  • By adhering to established/required guidelines and by familiarity and use of automated dispensing technology such as Pyxis, CII Safe and ParX.
  • PREPARES AND DISPENSES MEDICATIONS PROPERLY AND PERFORMS ALL OTHER TASKS AS ASSIGNED BY SUPERVISOR
  • By compliance with all stated/mandated, rules/regulatory requirements.
  • INSURES PROPER, CORRECT AND PROFESSIONAL CONDUCT IN THE ABSENCE OF THE DIRECTOR OF PHARMACY OR AN APPROPRIATE/APPOINTED DESIGNEE.
  • By oversight and assessment of compliance and mandated guidelines.
  • By remembering the hospital’s mission while interacting with other hospital employees or patients.
  • INSURES THAT ADEQUATE AND CORRECT INFORMATION IS DISSEMINATED TO APPROPRIATE PERSONNEL AND PARTICIPATES IN THE QUALITY IMPROVEMENT EDUCATION PROCESS AND IN DAILY QUALITY IMPROVEMENT ACTIVITIES AS DEFINED BY THE DEPARTMENT HEAD
  • By complying with stated designed, appropriate activities.
  • By documenting all clinical efforts.
  • By handing off communication to the next shift.
  • INSURES PROPER COMMUNICATION OF INFORMATION TO APPROPRIATE DEPARTMENTS
  • By applying established avenues/pathways of required procedures.
  • Calls physicians directly with any urgent matters, sends Theradoc messages for non-urgent but important issues.
  • Utilizes the “fax back” functionality or Rxe-view software when relaying order clarification information to the nursing department.
  • SUPERVISES NON-PROFESSIONAL PERSONNEL IN THE ABSENCE OF THE DIRECTOR OF PHARMACY OR AN APPROPRIATE/APPOINTED DESIGNEE
  • By oversight regarding requirements consistent with established guidelines.

QUALIFICATIONS:

  • Education
  • Degree in Pharmacy from an accredited College of Pharmacy required
  • Work Experience
  • 1-2 years related experience preferred
  • Licenses / Certifications
  • Pharmacist license required
