The CPS Solutions, LLC H ealth Equity and Systems Thinking Pharmacy Fellowship is aimed to position our graduates to improve health equity through advocacy, recognition of systems thinking, and adoption of community resources/partnerships to improve chronic conditions and address national crises, such as opioid use disorders. You will focus on addressing and improving healthcare disparities with an aim for a more culturally and competent pharmacist workforce in integrating social determinants of health into the medication use encounters and improving patient outcomes. This is a 12-month position only. Additional time may be added as needed and will be determined by the end of the term.

Schedule : This is an Exempt position and will work a minimum of 40 hours a week. Work will be done in the Dublin Corporate office and Pharmacy locations, to be determined.

Responsibilities:
  • Develop an assessment that incorporates structural and process of care indicators to improve health literacy and communication between pharmacy and patients during the medication management encounter.
  • Conduct a longitudinal research project for presentation at a professional platform.
  • Develop a structural Certificate Program in the area of health equity, disparity and systems thinking for preceptors and pharmacist leaders.

Qualifications:
  • Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE) with a minimum of a Bachelor's Degree. Doctor of Pharmacy degree preferred.
  • Post-graduate pharmacy residency (PGY1) or equivalent training.
  • Public Health degree or public health services experience preferred.
  • Licensure in good standing as a Registered Pharmacist in the state of practice.
  • Proficient computer skills using Pharmacy systems and Microsoft applications - Outlook, Word, PowerPoint, Excel.
  • Effective communication skills - verbal and written.
  • Through training and/or experience, possess necessary knowledge to provide oversight of medication regimens for the patient populations treated in the facility including a knowledge of dosing for specific age populations.
  • Complete application submission to include curriculum vitae (CV), personal letter of intent and three letters of recommendation or reference.
