POSITION SUMMARY:

The Home Health Physical Therapy Assistant (PTA) will provide physical therapy services according to accepted standards of treatment to patients in the home, as ordered by the attending physician and under the direction of the physical therapist.

ESSENTIAL FUNCTIONS: To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation; including, but not limited to:

  • Treat patients to relieve pain, develop and restore function and maintain maximum performance using appropriate treatment, modalities and procedures as outlined with plan of care
  • Document and report to the physician the patient’s response to treatment and/or any changes in the patient’s condition
  • Instruct company visit staff, patients and patient representative(s) in the use of prosthetic, orthotic, assistive devices and home exercise programs
  • Actively participate in case conferences and communicates effectively with all those providing care
  • Perform and document pertinent patient/patient representative(s) teaching of each visitor to the plan of care and use of therapy devises, home regime, safety and attainable goals; teaching promotes patient independence and self-care
  • Promote teamwork and helps with coverage for scheduled and unscheduled time off
  • Complete visit notes and documentation daily documenting therapy provided and submits documentation timely as outlined by company policy
  • Assist with the orientation process of new staff members if needed
  • Must be willing to travel to other office locations to assist with staffing if needed

Required Skills:

  • Work requires advance knowledge of the Homecare Industry, Medicare Regulations, CMS, and other State/Federal regulatory division

Required Work Experience:

  • One (1) year experience as a Physical Therapist Assistant preferred
  • Home Health experience preferred

Licensing/Certification/Education Requirements:

  • Graduate of an accredited Physical Therapy Assistant Program
  • Follow all Regulations pertaining to the State Practice Acts and follows the American Physical Therapy Association Code of Ethics
  • Current CPR Certification
  • Current Physical Therapist Assistant License

Other Requirements:

  • Position may require unscheduled overtime, week-end work
  • Must be willing and able to work flexible hours
  • Travel required
  • Must have reliable transportation
  • Current State Driver’s License
  • Current Auto Liability Insurance according to Company Policy

Physical Requirements:

  • Frequent lifting of objects up to 40 pounds
  • Subject to standing, walking, sitting, bending, reaching, kneeling, pushing and pulling
  • Work is performed in a standard home health environment

Supervisory Responsibilities:

  • Does not have direct supervisory responsibilities
