The Specialized Treatment for Optimized Programming (STOP) Program Area Six connects California Department of Corrections and Rehabilitation inmates and parolees to comprehensive, evidence-based programming and services during their transition into the community, with priority given to those participants who are within their first year of release and who have been assessed to as a moderate to high risk to reoffend. Area Six includes San Diego, Orange, and Imperial counties. STOP subcontracts with detoxification, licensed residential treatment programs, outpatient programs, professional services, and reeentry and recovery housing throughout the program area to assist participants with reentry and recovery resources.
Placement Coordinators is responsible for placing clients in available treatment slots with Community Based Providers (CBPs). Providing high quality customer service, Placement Coordinators receives referrals for inmates from direct prison placements and parolees within the community including the county jail system and local Parole offices. Confirm treatment slot availability with CBPs and coordinate release dates with transportation services as needed. Accurately documents placement notes in electronic system as required by California Department of Corrections and Rehabilitation (CDCR) and HealthRIGHT 360.
The Placement Coordinator staff member will be tasked with, but not limited to; monitoring California Department of Corrections and Rehabilitation (CDCR) subcontracted community based direct service treatment programs and housing sites. Physical site visits/inspections as needed. They will also be tasked with ongoing reconciliation reporting, tracking, placement, monitoring subcontractor fidelity to EBP curricula, subcontractor reporting accountability, quality of food, hygiene and facility maintenance and compliance with gender-responsive, trauma-informed and culturally diverse contractual requirements. Responsible to independently perform issue investigation and tracking, in-depth research, root-cause analysis, resolution identification, and proactive data monitoring to ensure the utmost integrity of HealthRIGHT360 data. Will develop and maintain working knowledge of overall compliance, and reporting through regular inspection of records and reports. The Placement Coordinator staff member may also be tasked with monitoring the accuracy data entry into the Automated Reentry Management System (ARMS).
Key Responsibilities
Placement Responsibilities: Receive referrals for parolees from the in prison programs and DAPO Parole Units as needed. Proactively links clients to CBPs based on their treatment needs/ level of care. Submits service authorizations to CBPs. Assess, schedules and coordinates placements of project referrals into appropriate community placements. Coordinates transportation with Transportation Coordinator. Matches inmates/parolees to the appropriate level of care within a network of sub-contracted community based providers. Screens profile for possible barriers to treatment delivery. Receives transitional plans from CDCR and County institutions. Conducts daily communication with Parole Agents. Conducts presentations to community agencies to increase pool of referrals. Identifies community capacity needs. Revises and submits placement requests. Conducts presentations to community agencies and for the purpose of technically assisting CBP staff. Attends required trainings and meetings and assist in training of new hires. Serve as an Incident Reporting designee on as needed basis. Other duties as assigned. Placement responsibilities are considered to be the primary focus of this position.
Administrative Responsibilities: May be tasked with coordinating client release dates with transportation services. Ensures ongoing communication with the CDCR personnel and community based subcontractor staff regarding participants. Maintains and updates bed counts and/or reporting spreadsheet(s) and may submit reports to the CDCR as required. Attends required trainings and meetings and assist in training of new hires. Serve as an Incident Reporter on as needed basis. Keeps track of received data and source documents. Prepares and sort source documents, and identify and interpret data to be entered. Performs general clerical duties such as typing, answering phones, filing etc. Process various forms. Generate reports and respond to inquiries regarding entered data as requested. Attends required training and meetings. Communicates with California Department of Corrections and Rehabilitation (CDCR) personnel. Maintains project documents and records. Submits reports to QA/Fidelity Supervisor, Case Manager Supervisor, and/or Program Director as needed. Conducts ongoing communication with a large network of subcontractor’s serving the population.
Documentation Responsibilities: Notifies AORs of participant placement. Enter placement s notes into data and/or ARMS Data system as needed. Coordinates with available internal and external resources (institutional SAP staff, Parole, Community Transition Program (CTP), community based providers, County Jails, STOP staff, etc.) to produce placement notes. Maintains accurate records by data entering documentation into various electronic systems for clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
Creates, opens, updates, closes and archives client files in accordance with department procedures. Assist in establishing and maintaining an effective and efficient records management system. Ensures participant files meet contractual compliance. Maintain and comply with confidentiality requirements. Conducts file compliance and reconciliation. Contacts preparers of source documents to resolve questions, inconsistencies, or missing data. Reviews and make necessary corrections to information entered. Other duties as assigned. Compiles, sorts, and verifies accuracy of data to be entered.
Education and Knowledge, Skills and Abilities
Education and Experience (Required):
- Minimum of two (2) years’ experience working with criminal justice and/or substance abuse treatment programs.
- Experience working in a customer service driven environment.
- Two years of college education in the field of criminal justice, psychology, sociology, or related field.
- Experience using an electronic health record system.
- Experience with behavioral and/or physical health data.
- Knowledge of co-occurring disorders and trauma informed treatment.
- Knowledge of Clinical documentation.
- Experience with governmental behavioral health contractual and/or sub-contractual standards.
- Must undergo a background investigation and criminal records check to obtain clearance.
- Must be able to satisfy requirements for security clearances.
- Must not be on active parole or probation.
- Advanced Microsoft Excel data manipulation/analysis knowledge.
- Culturally competent and able to work with a diverse population.
- Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.
- Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency.
