Job Summary: The Population Health Administrator is responsible for oversight of Integral Care's accountable care and pay-for-performance initiatives. The Population Health Administrator works closely with the Director of Practice Management and Chief Operations Officer to ensure the delivery of quality behavioral health services (in accordance with Certified Community Behavioral Clinic (CCBHC), 1115 Waiver, NCQA and managed care standards).

The Population Health Administrator is responsible for leading the design, implementation, and support of a robust agency wide population health management program. This position should identify and recommend technology innovations and opportunities to expand and customize the reporting and data analytic platforms to meet stakeholder data needs, stratify patient populations by risk and drive client outcomes.

This position will develop analytical recommendations for holistic value-based care in a changing healthcare environment. The Population Health Administrator will proactively involve and collaborate with Integral Care program leadership, health promotion teams, prescribers, manage care organizations and local partners in identifying and improving patient experience, costs of care and health outcomes.

Key Responsibilities:

  • Design and implement a population health program to influence client care and improve health outcomes; specifically establishing the risk stratification tools and client priority listings to direct and guide the newly formed health promotion team's work
  • Recommend point-of-care and dashboard solutions to improve agency wide performance and clinical decision making
  • Develop strategies for program evaluation in order to demonstrate value to internal and external stakeholders, including payers and local partners
  • Demonstrate excellent communication and presentation skills with the ability to engage and coordinate staff, community stakeholders and executive leadership

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Master's degree.

License:

Required:

  • Current and valid driver's license and a clean driving record.

Hours: Full-time; Workdays; Monday - Friday Workhours; 8:00AM - 5:00PM

Salary: $96,656.00 annually
