Responsibilities

Job Summary:

Under the direction of the Program Manager MAPC his position will service as the team lead for: increasing public awareness of the PD and the service of the Muhammad Ali Parkinson Center (MAPC) within the Hispanic communities; planning, developing and delivering community based education and outreach programs for the Hispanic communities with a culturally sensitive approach; developing strategic collaborations with community based organizations serving the Hispanic community.

Community Support/Programs

  • Serves as a key liaison between consumers, physicians, Program Manager and Center for all inquiries and services pertaining to the Hispanic community.
  • Provide education to the Hispanic community by info/education sessions to care providers e.g. Physicians, nurses etc., participate in health and community sponsored health fairs and programs and provides educational materials and information to patients, families and the health community.
  • Determine the needs of people with Parkinson's disease and their families in the Hispanic community.
  • Plan, develop, organize, coordinate and conducts education and outreach programs for the Hispanic communities to empower Hispanic families living with Parkinson's disease e.g. educational seminars, creative workshops, voice groups
  • Plan and coordinate annual educational symposium in Spanish to bring the latest developments in living with Parkinson's Disease to the Hispanic community
  • Increase community awareness and public relations relative to the Hispanic community
  • Participate in health and community sponsored health fairs and programs
  • Attends community resources meeting and meetings with community agencies
  • Takes a lead role in the Expressive Arts program by assessing, developing and coordinating expressive arts workshops to meet the needs of the Parkinson community
  • Serves as a bridge between the MAPC and Spanish speaking Parkinson Disease organization outside of Arizona

Patient Care Services

  • Administer, score and discuss results (with patient and family) of patient assessment scales
  • Demonstrates critical thinking skills in client evaluation to anticipate issues/needs and proactively recommend intervention
  • Supplies educational materials and information to patients, families and the health community to allow them to maximize their care
  • Plan, develop and facilitate support groups as needed and assist existing support groups to locate and acquire speakers, mail out notices, and facilitate groups as needed.
  • Work in collaboration with the Movement Disorders Clinic to facilitate quality care for Hispanic patients. Assist with communication problems or issues relating to quality of care
  • Lead for development and supervision of a Promotores program to the Hispanic Parkinson's disease community
  • Attend and contribute to team meetings
  • Prepares/translates educational and promotional material for the newsletter, calendar, media and other related publications newsletter, calendar, media and other related publications

Qualifications

Minimum:

Experience:
Five years of experience in clinical care/rehabilitation preferably in neurology and/or geriatrics.
Prior experience interacting with the public and other healthcare professionals is a necessity.
Spanish and English are required.

Education:
Bachelor's degree in within a healthcare discipline required

Licensure:
Gerontology certification preferred.

Special Skills:
Skills in critical thinking, effective communication, conflict management, problem solving, autonomy and decision making are highly desirable.
