Position Description:
The Ryan White Kentucky Care Coordinator Program Manager (KCCP) is responsible for the overall operation of Ryan White Part B support services program which primarily includes non-medical case management (case management services), emergency support, and medication assistance via KADAP for people living with HIV (PLWH) in the community. This position encompasses financial oversight of the program grant, supervision of case managers, and ensuring the program meets all state and federal requirements.

$85,798.00
Target Compensation Minimum:
$48,421.00
Compensation will be commensurate to candidate experience.

Position Description:

This position encompasses financial oversight of the program grant, supervision of case managers, and ensuring the program meets all state and federal requirements.
Essential Duties and Responsibilities
- Responsible for services administration for $2.7 Million Ryan White Part B grant in support of 2000+ Ryan White eligible clients in care in the KIPDA region.
- Supervises two (2) Sr Medical Case Managers and a team of twenty (25) Case Managers.
- Work collaboratively with Senior Case Management team to establish high level priorities based on weekly client level assessment reports.
- Ensures case management workflow adheres to state mandated performance levels of less than 80 clients per case manager.
- Ensures case manager care plans, referrals and other performance indicators are on target with state and federal achievement standards
- Work closely with financial team to review monthly financial reports and ensure timely submission to appropriate University, state, and federal agency.
- Assists Program Director in the development of quality improvement projects designed to improve overall care delivery.
- Assists Program Director in program quality assessment to ensure quality care or access, via review or development of policies, procedures, protocols, or educational training sessions.
- Responsible for writing quarterly grant quality reports for state submission and review.
- Ensures that data management and statistics for required reporting and program evaluation is performed and submitted in a timely manner.
- Works collaboratively with HIV/AIDS Branch of KY Cabinet for Health and Family Services, Division Ryan White Program Director and 550 Clinic RW Quality Management team to identify, create, administer, and facilitate training opportunities for UofL and Ryan White staff.
- Communicates effectively with colleagues across departments, patients and/or family members when appropriate.
- Ensures all university, state and federal policies and procedures are adhered to as it applies to the administration of the Ryan White Part B KCCP.
- Other duties as assigned.
