Hearth is a non-profit organization dedicated to ending homelessness among the elderly.

The Program Nurse is a case management position, not a clinical nursing position. Hearth has two openings for two part-time 25 hours minimum per week - no nights or weekends required but will consider if applicant prefers. The Program Nurse will work with residents of Hearth's supportive housing site at Olmsted Green in Dorchester as well as one small congregate housing site in Jamaica Plain. This position works as part of an interdisciplinary clinical team to provide care coordination of health services to residents, many with chronic health and behavioral health issues. The focus is case management of health, behavioral health and wellness services through the Group Adult Foster Care program and oversight and supervision of paraprofessional staff. This is not a clinical nursing position.

Major Functions:

Assessment

  • Participates in assessment and interdisciplinary review of care
  • Works with and assists clients to reach goals outlined in care plans
  • Makes initial nursing assessments on all clients, and reviews and updates those assessments regularly
  • Develops care plans promoting effective services
  • Coordinates service plans, and participates in ongoing review of the plans' efficacy in conjunction with professional assessment team members
  • Updates service plans every six (6) months coordinating with a multidisciplinary team
  • Participates in the discharge planning process, helping to arrange for services when needed
  • Provides consultation services to colleagues individually and in groups in the areas of mental and medical health
  • Provides ongoing health information and education to clients about their illnesses, medications, and wellness issues.
  • Follows up on health educational presentations (such as heat stress, diabetes)
  • Communicates important medical issues to the program staff and educates the staff in the management of these issues
  • Acts as liaison with area hospitals, community agencies, and treatment providers and participates in case conferences.

Supervisory

  • Directs and supports the Personal Care/Homemaker in the execution of personal care tasks

Reports to

  • Director of Health Services

Qualifications:

  • Clinically grounded RN. BSN preferred.
  • Massachusetts Licensed Registered Nurse in good standing.
  • Minimum of two years’ experience required including some acute care experience
  • Community health and/or mental/behavioral health experience strongly preferred

Additional Required Skills:

  • Thorough knowledge of area’s health and social service agencies
  • Ability to clearly communicate concepts, ideas, instructions, etc. to others in varying levels of vocabulary, both verbally and in print
  • Close attention to detail with an expert ability to multitask
  • Ability to maintain therapeutic boundaries under extreme stress
  • Communicate with interdisciplinary team
  • 1 year supervisory experience preferred

Schedule:

25 hours minimum per week Monday through Friday. (No nights or weekends required, but if the applicant prefers, we can accommodate 1 weekend day)

Salary:

$36.05 per hour plus full benefits

Work Location:

  • Main office is at 2 Kingbird Rd, Dorchester
  • Nurse will visit/meet with residents at the Bishop Street home in Jamaica Plain a few hours a week. There are a total of 9 female residents at this home.
