Job Title: Program Specialist
Department: Community Health Action Department
Supervisor: Senior Manager of Prevention Programs & Technology Innovation Initiatives
Location: This position will be primary located in DC; but need to be able to move between locations as needed. This position will work remotely and on-site, as needed.
Classification: This is a non-Exempt/Hourly position
Synopsis:
This position will work within the Community Health Action Department to provide programmatic technical support and coordinate interventions through the development and implementation of participatory health education methodologies, peer-based support groups, peer health promoters training, individual health education, psychoeducation sessions, and navigation services to promote awareness about prevention-related topics within the Latinx community.
This position will also support the program manager in developing and maintaining collaborations with other community organizations, educational institutions, and providers that serve our target populations, as well as coordinate program data collection and reporting initiatives with Program Data Monitoring Specialist.
Qualifications: Required Education and Experience:
- Bachelor’s degree in health field, or related discipline, or an equivalent three-year combination of education and experience from which comparable knowledge and skills may be obtained.
- Bilingual (Spanish-English) / bi-cultural.
- Three years’ experience working in service delivery with the Latino immigrant community and/or multi-ethnic community.
- One year experience working with sexual minorities, youth, and people living with HIV within the Latinx LGBTQ community particularly in safe spaces settings.
- Three years of experience with community health educational strategies, including the design of educational materials, curriculums, and charlas.
- One year of experience implementing educational and psychoeducational interventions.
- Comprehensive knowledge of social determinants of health, learning, and behavior change theories.
- Knowledge, skills, and experience in implementing Hight Impact Prevention and Behavioral Change interventions.
Preferred Education and Experience:
- One year of experience implementing educational and psychoeducational interventions for people living with HIV.
- Two years of experience working with community health workers.
- Two years of experience in maintaining and enhancing networking efforts with community stakeholders for navigation services.
- Experience submitting and reporting programmatic expenditures according to budget, following financial guidelines.
- Experience implementing participatory research methodologies including dissemination efforts.
Required Skills/Abilities/Certifications/Licenses:
- High motivation for work in public health field.
- Excellent oral and written communication skills in both English and Spanish.
- High motivation to advocate for sexual and racial minorities, women, people living with HIV, and LGBTQ communities’ rights.
- Strong ethical core.
- Computer literate in Microsoft Office, including electronic medical records (EMR) or other database software knowledge preferred, as well as ability to navigate and search for information on the World Wide Web/Internet (www).
- Leadership, interpersonal, problem-solving, detail-oriented, teamwork, self-starter, and effective organization skills.
Preferred Skills/Abilities/Certifications/Licenses:
- DC, MD, or VA Community Health Worker Certification.
Duties and Responsibilities:
- Supports the development and implementation of program initiatives and interventions, through:
- Designing educational strategies and interventions to promote a positive sexual health and sexual rights.
- Supporting the program’s development and implementation of programmatic reports, new initiatives, and quality assurance efforts, as required.
- Leading the program’s design and/or update of educational curriculums and programmatic protocols/ guidelines/ workflows, as required by program manager.
- Coordinates the program’s navigation system to link clients to appropriate resources to meet needs and goals, including the identification and scheduling of interpretation, housing, legal, job-related, education and other services as needed through:
- Conducting on-going assessment, monitoring, triage, and reassessment of client progress and needs.
- Developing individual plans to navigate clients to the identified services needed.
- Assigning staff and/or peer health promoters to conduct the client navigation planned.
- Maintaining client records and other data bases with client information assuring completeness, filing, neatness, confidentiality, and documentation.
- Coordinating client support with care team coordination within La Clinica and provide referrals, as needed.
- Building and maintaining strong networking activities with community service providers and La Clinica’s care teams.
- Conducting quality assurance processes by reviewing and providing feedback on the navigation’s plans created for the health educators and navigators to link clients to care and social support services.
- Coordinates the program’s implementation of risk-reduction and health education interventions, through:
- Supporting program manager in the design and implementation of strategies to recruit and engage high-risk clients for HIV and people living with HIV and provide ongoing, multi-session interventions for risk-reduction and behavioral change.
- Developing of an individualized prevention plans with goals and measurable objectives for clients and support the rest of the team in the creation of the prevention plans with their own clients.
- Coordinating the delivery of culturally appropriate Program’s health education interventions through selection of topics and agenda, development of educational tools for each session, and individual and/or group sessions facilitation.
- Coordinating the implementation of weekly support group meetings and individual psycho-educational sessions.
- Supports program manager to implement training program to enhance staff capacity, including but not limited to, the implementation of the program’s workplan for health promoters training, through:
- Training design and planning process, including supporting the integration with Department’s annual trainings for health promoters.
- Implementation of staff training sessions (including promotores) and facilitation.
- Supports Program Manager in Program Development & Integration initiatives, through:
- Supporting and attending Program and Department health educators case discussion meetings.
- Supporting program manager in the integration of LGBTQ sexual health strategies with the rest of the programs within the CHA Department and La Clinica.
- Supporting program manager in the design and implementation Department’s annual trainings for health promoters, as needed.
- Supports program manager in community collaboration efforts, through:
- Coordinating participation in local, regional, and national coalitions.
- Supporting program manager and/or Department Director in implementing participatory research methodologies including dissemination efforts.
- Supports program manager in other general duties, such as:
- Coordinating execution of assigned program’s expenditure budget including expenditures reporting, as planned by program manager, ensuring spending according to approved budget and Finances and Grants requirements.
- Coordinating programmatic workplan of interns, as appropriate.
- Represents La Clinica by attending conferences, meetings, coalitions, and community planning groups meetings, as required.
- Attends required trainings to increase/maintain skills.
- Meets with supervisor as required to coordinate implementation of programs and services.
- Attends program, department, and all staff meetings, including all mandatory La Clinica activities.
- Follows La Clinica’s Code of Conduct, and all other personnel Policies and Procedures.
- Performs other related duties, as assigned.

Physical Requirements:
