Position Summary
The Division of General Pediatrics and Adolescent Medicine provides primary and general care to children and adolescents at the North Carolina Children’s Hospital and conducts research and education on related topics.

The Transition Care Management Program within the division is a program designed to improve family-reported care quality for children with medical complexity, reduce subsequent acute-care use, facilitate connection between specialty care and community-based primary care, and expand the scope and enhance the spread and sustainment of complex care both at UNC and around the state.

This position will provide day-to-day operational leadership for Transitional Care Management Program activities, oversee the collection and analysis of relevant data, and help to develop products of scholarship (presentations, manuscripts, grant proposals) related to this work.

This position will support and lead the program to achieve the stated goals and to collaborate with other services and clinicians both inside and outside of the hospital. This work will need to be conducted independently, therefore, the Program Specialist must be comfortable navigating moderately complex healthcare delivery systems.

This position will organize, manage program data, supervise implementation of activities by others, and change processes based on standard improvement science principles. The Program Specialist will be responsible for collecting and analyzing information needed for the program and the program database. In addition to supporting related quality improvement, programmatic, and research activities involving the complex care program.

Organizing and directing transitional care management work of the UNC Children’s Complex Care Program will be a primary responsibility, this individual or others in the program will meet the family as patients are preparing to leave the hospital and communicate the Transitional Care Management program contact information, complete screening forms for medical complexity, and communicate with clinical team members in Complex Care about transition care plans for the child and family. Furthermore, this position will help ensure that nurse follow-up calls are scheduled and take place for discharged patients and will help manage others who are performing these activities, all the while independently managing the multifaceted program within the established guidelines, ensuring accountability.

The University Program Specialist will work closely with the Complex Care Program Medical Director to coordinate related activities, including IRB applications, recruitment and data collection procedures, grant applications, and assist with the preparation of scholarly projects such as presentations and manuscripts.

Additionally, this position will work closely with the grant manager within the department, to evaluate and manage business operations and the financial status of the program; analyze data and present it to funders and other hospital programs with shared goals; engage all involved team members in outpatient and inpatient general pediatrics teams, contribute to Complex Care Team biweekly meetings, and creatively improve the program methods as needed to achieve goals.

The project and this position are funded by a grant from the Innovation Health Foundation. This is a time-limited position for 2 years due to the nature of grant funding.

Minimum Education and Experience Requirements
Bachelor’s degree and one year of experience related to the area of assignment; or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.

Required Qualifications, Competencies, and Experience
  • Some quality improvement training and data analysis necessary.
  • Professional knowledge of healthcare systems and data management.
  • Must be comfortable navigating moderately complex healthcare delivery systems.

Preferred Qualifications, Competencies, and Experience
  • Master of Public Health or Master of Healthcare Administration strongly preferred.
  • Demonstrated ability to coordinate and administer program activity and quality improvement initiatives.
  • ability to act as a consultant for this and other similar programs independently around the region and country.
  • Experience analyzing, reviewing, and summarizing data.
  • Experience presenting information and data verbally and in written form, including presenting information to groups of people.
  • Demonstrated ability to work collaboratively with coworkers, patients and families, and community agencies.
