PROLOGUE JOB DESCRIPTION

Position Title: PRP - Rehabilitation Direct Care

General Description: The RDC provides daily services to adults recovering from mental illness and/or substance use disorders. The RDC will work with individuals, assist them with developing their service plans, provides direct psychiatric rehabilitation services and provides skills training. The RDC works with the individuals we serve, individually and in groups to assist them in choosing and obtaining personal goals and developing the skills necessary for independence and success.

FLSA: Non-exempt

Required Education, Knowledge and Skill Abilities to Perform Essential Job Functions:

An AA degree with at least two (2) years experience in mental health/psychiatric rehabilitation/related human services or a Bachelor's degree is preferred. Ability to work as a team player is necessary. A good driving record is required and is defined as having a valid driver's license with no more than 1 point. Ability to respect individual choice in the recovery process is required. Also, this position requires good verbal and written communication skills, intermediate computer literacy and the understanding to respect the abilities and limitations of the individuals we serve.

Summary of Essential Functions, Responsibilities and Duties

• Facilitate and write initial assessment
• Assist individuals develop their Individual Service Plans and complete the written plan
• Write monthly progress notes
• Facilitate groups and write appropriate lesson plans
• Participate in assigned Service Team
• Communicate with the Rehabilitation Team Coordinator to ensure continuum of service
• Provide program coverage in all areas and in all aspects, including on site services and documentation
• Provide off site services, including in the home
• Perform medication management
• Utilize Electronic Health Record to record services and documentation
• Utilize computer and systems for external and internal communication, time and attendance
• Other duties as assigned

Essential Physical Requirements Include:

• Drive Prologue vehicles which may include cars, mini-vans, mobility buses
• Assist clients to get into offices for appointments (somatic, psychiatric, Social Security, etc.)
• Physically assist clients with their personal mobility by working wheelchairs, assisting with walkers, etc.
