POSITION SUMMARY:

The PRSD is responsible for providing related social work services so that each resident may attain the highest practicable level of physical, mental and psychosocial well-being. The person holding this position is delegated the responsibility for carrying out the assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures.

ESSENTIAL JOB FUNCTIONS:

A. Role Responsibilities – Job Knowledge/Duties:

1. Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.

2. Demonstrates knowledge, skills, and techniques necessary to plan social services for residents with the following needs; therapeutic, neuro and dementia, dialysis, IV therapy, and infectious diseases if applicable

3. Attends all mandatory in-services by the employee’s anniversary date

4. Acts appropriately under the direction of the Administrator and acts as an active member of the interdisciplinary team

5. Demonstrates ability to adjust to changes in unit/shift assignments to meet resident and family needs

6. Communicates and observes the Corporate Compliance Program effectively and complies with Code of Conduct when performing work functions

7. Other duties as assigned by the Administrator

B. Role Responsibilities – Administrative Duties & Resident Care

1. Supervises, trains, evaluates, counsels, and disciplines inter-departmental personnel

2. Assists in the recruitment, interviewing, and selection and termination of social services personnel

3. Determines departmental staffing requirements necessary to meet the Social Services Department’s needs and recommend to the Administrator the number and level of ersonnel to be employed

4. Schedules department work hours (including vacation and holiday schedules), personnel, work assignments, etc., to expedite work and stay within approved budget

5. Provides orientation to long term care facility and services rendered, to residents and family members

6. Arranges inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement

7. Schedules appointments with residents or responsible party, guardian, next of kin, sponsoring agency or agencies or lawful representative to:

a) Review and interpret Resident’s Rights, including facility Policy and Procedure implementation. Obtain signatures, sign as a witness to signatures, and provide client or above-mentioned representative with copies

b) Review and explain facility Advance Directives policy. Obtain signature, sign as witness to signature, then provide residents or representative copies of DNR regulations, rights, duties and responsibilities

EDUCATION/EXPERIENCE:

1. A Master’s Degree in Psychology, Sociology, Social Work or related field

2. Possess a valid State Driver’s License and have transportation available for job related travel

3. Must be capable of performing the essential functions of the job, with or without reasonable accommodations

5. Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required
