Position Summary
The quality assurance team member is responsible for reviewing all clinical records regarding compliance with regulations, OASIS, quality of clinical documentation, coordination of the multidisciplinary care plan, and development of the plan of care signed by the physician, including identification and implementation of staff education and training to achieve superior clinical outcomes and quality of care.
Key Responsibilities
- Completes clinical record review at start of care, resumption, re-certifications, and discharge for complete and accurate documentation, development of appropriate discipline-specific care plan, accurate physician orders, and coordination of care and compliance with Medicare, Joint Commission and state regulations.
- Completes records reviews within agency-defined timeframe
- Demonstrates understanding of current Medicare guidelines and ensures that clinical records are compliant with governing entities; maintains agency in survey-ready status.
- Provides expertise in OASIS, record review and staff education.
- Collaborates with Coder to ensure accuracy of OASIS and develop educational strategies to optimize outcomes.
- Maintains proficiency in OASIS dataset and data capture best practices, according to industry guidelines and CMS instructions.
- Assures agency compliance with OASIS requirements.
- Compiles findings and creates reports as assigned.
- Facilitates the activities of the QAPI committee.
- Knowledgeable on PDGM current policy.
- Pre-biller on charts.
- Additional duties and responsibilities may be assigned.
Position Qualifications:
- Graduate of an accredited practical nurse or vocational nursing program.
- Currently licensed as LVN in the state of California.
- Minimum of two years of professional nursing experience in Home Health with QA experience required.
- Possess and maintains current CPR Certification.
- Demonstrates proficiency in use of the DSL (Data Soft Logic) and other available web-based programs.
- Good verbal and written communication, organization and public relations skills.
- Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
Job Type: Full-time
Pay: $20.00 - $24.00 per hour
