JOB DESCRIPTION SUMMARY

The Referral Management Specialist supports all aspects of the referral management process including, but not limited to, benefits verification, clerical duties and coordination with the marketing department to obtain sufficient supporting clinical documentation and complete orders for services.

QUALIFICATIONS

  • High school Diploma or high school equivalent, required.
  • One (1) year experience or more working in a related field/position in Home Health.
  • Demonstrated ability to work with Microsoft Suite and home care/hospice software; Kinnser experience preferred.
  • Ability to communicate tactfully with customers and the community.
  • Demonstrates autonomy, organizational skills, assertiveness, flexibility and cooperation in performing job responsibilities.
  • Access to reliable transportation.

ESSENTIAL JOB FUNCTIONS

  • Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake
  • Participates in the on call schedule for referral/scheduling departments.
  • Create and maintain physician profile in Kinnser software.
  • Maintain master list of clinicians including contact number and territory
  • Coordinates with marketing to ensure referral sources match in Kinnser and Playmaker.
  • Routine clerical duties as needed including answering phones.
  • Responsible for producing the Surgery Schedule
  • Maintains accurate census count in Kinnser by reviewing active charts weekly.
  • Maintains and updates the Physicians binder as needed.
