About the Role

Responsible for direct and indirect care given to assigned participants in compliance with state regulations, professional standards of practice, physician orders, and individual health/treatment plans. Conducts nursing assessments and identifies health needs of Class Members. Administers and monitors prescribed medications and treatments in accordance with approved nursing techniques and physician’s orders. Provides case management to persons with severe mental illness or co-occurring disorders, both on-site and in the community. Recommends and refers participants to appropriate medical services. Documents all services using program standards. Acts as a collaborative member of an interdisciplinary team in agency. Provides back-up coverage to other agency nurses.

Requirements

• Nursing degree from accredited university/college.
• Experience working with persons with psychiatric disorders and dual-diagnosis population required.
• Valid Illinois Registered Nurse (RN) license required.
• Valid Illinois driver’s license, insurance, and daily access to vehicle.
