ApolloMed is looking for a Registered Nurse - Hospice RN to work at our Rosemead, CA location at 3505 Hart Avenue. You will be required to travel to patient homes but will be reimbursed at the IRS mileage rate. This position's schedule is typically Mon-Fri from 9am to 5pm but you will also need the ability to work different hours if there is patient need.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Completes initial, comprehensive and ongoing assessments of patient and family to determine hospice needs
- Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions
- Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given
- Communicates with Director of Nursing about the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required
- Participates in on-call duties as defined by the on-call policy

REQUIRED:
Active and unrestricted RN license in California- CPR certification
- Ability to drive to client sites in the state of California
- One (1) year of clinical experience
