This position is heavy on patient communication via telephone; there is not a great deal of in-person patient interaction. This role also entails heavy amount of computer work as many essential job duties are completed via the practice’s Electronic Medical Record system. The ideal candidate will enjoy patient coordination of care, case management duties, as well as administrative duties, but not require regular in-person patient contact to feel job satisfaction. Our staff and providers rely heavily on our Nurse Navigator Team for daily support; the RN team is invaluable to our patient care team.

This position will work four 10-hour days per week, Mon-Fri.; and may be schedule any day of the work week. No night or weekend shifts required. We are seeking a candidate for our office location at the North Suburban Medical Center in Thornton, CO. This position is not remote and work is performed on location.

Responsibilities and Duties:

Each RN works independently at a designated clinic under the clinical direction of that office’s specific providers. The RN team additionally provides cross coverage of other office’s providers and patients as needed on a daily basis. This requires a team approach and high degree of communication shared amongst the team to keep workloads balanced.

  • RN navigator/case manager of CKD patients through our practice’s pathway of renal care, aka In-Track
  • Monitoring reports and placing patient In Track
  • Education on dialysis modalities
  • Collaboration with provider and interdisciplinary healthcare team to ensure patient is educated on renal replacement therapy (RRT)/end of life to make an informed decision
  • Tracking referrals for education, access placement and follow-up, transplant, palliative care, and hospice
  • Tracking patient compliance
  • Assessing for barriers to care
  • Working closely with providers to improve efficacy in patient care with the goal of ensuring optimal RRT starts.
  • Coordinating successful transition from office to RRT
  • Updating the In Track tool to assist with data reporting
  • Managing challenging behaviors from patients and/or family members, including possessing knowledge about the psychosocial issues’ patient/families face related to CKD/RRT
  • Maintenance of incoming EMR messages regarding patient care issues, and timely response and follow-up
  • Educate and maintain contact with patients to ensure anticipated therapeutic objectives are being met (i.e., weight/edema; BP; diet changes (low/high dietary K+, Na, phosphorus, Ca, lab work/diagnostic tests) and communicate with treating providers regarding objectives
  • Review providers’ electronic inboxes when they are off to assess lab and diagnostic testing results and determine what is routine versus what items need immediate attention and review by another provider. Communicate with covering providers to take necessary action with the patients to ensure timely care.
  • Entry, review, and maintenance of medications in EMR – includes Rx refills and med list reconciliation for office patients when appropriate
  • Medication pre-authorization for office patients via CoverMyMeds, contacting insurance provider directly and/or arranging appeals, creating letters for medical necessity, applying for medication/copay assistance programs, working with specialty pharmacies.
  • Anemia Management - Monitor labs per protocol and collaborate with provider for EPO dose adjustments. Communicate dosage changes to infusion center, and assist with sending updated prescriptions as needed
  • Kaiser Patient Care à entering orders for labs, infusions (Venofer/Rituxan), EPO, monitoring Kaiser email/communication with Kaiser team – i.e., Pre-ESRD coordinator, infusion coordinator, etc., vascular scheduler, transplant, anemia management, etc.
  • Assist with finding resources for patients – prescription assistance programs, meal program services, transportation services, medical supply – i.e., wheelchairs, walkers, home IV fluid supplies, home health care, palliative care, and hospice
  • Assist with research department studies – assessing patient candidacy based on provided criteria as requested by provider and communicating with the research team on possible patient candidates
  • Work collaboratively with the practice Clinical Quality Officer in updating protocols and principals of practice as needed and keeping our documentation for these up to date in our RN Team Manual
  • Maintain skill level and knowledge base through continuing education
  • Other duties as assigned


Required Experience:


  • Graduated from an accredited school of nursing
  • Colorado State RN license in good standing, and current CPR certification
  • Minimum 5 years of current nursing experience in a correlated field required (med/surg, IM, critical care, cardiology, urology).
  • Nephrology/dialysis experience a plus but not required
  • Office-nursing experience (including heavy triaging/navigating chronic disease) preferred, not require
  • Communicate verbally and in writing in a precise, clear, and professional manner
  • Skilled with prioritizing and multitasking; excellent critical thinking
  • Knowledge of Fluid/Electrolyte and Acid/Base physiology
  • Knowledge of various lab panels: BMP/CBC/RFP/LFT, etc.
  • Communicate verbally and in writing in a precise, clear, and professional manner
  • Compassion for patients and the desire to positively contribute to their health status
  • Possess and display a high level of patience and professionalism in all situations
  • Proficiency with using an EMR system for patient health record maintenance and patient communication
  • Effectively and professionally prioritize and multitask in a fast paced, sometimes stressful, environment
  • Possess an intermediate skill level with Microsoft Word, Excel, and Outlook, and website navigation
  • Ability to function at a high level of efficiency without supervision
