We are looking for a compassionate, detail-oriented Registered Nurse to join our team as a full-time RN Case Manager. Our Case Managers oversee all (home-based) skilled nursing care based on the plan of care for patients located throughout the Glen Allen, Shortpump and Surrounding areas. You will complete and submit Oasis Assessments, Transfers, Resumption of Care, Discharges, and Significant changes in condition in accordance with Agency time frames. The Registered Nurse will effectively interact with patients, caregivers, family members, and other interdisciplinary co-workers to provide skilled nursing care to patients on an intermittent basis in their place of residence. We offer a $10,000 sign-on bonus +competitive salary and an amazing bonus structure (six-figure potential) + mileage reimbursement

Required Qualifications:

• Graduate of an approved school of professional nursing
• Current and valid Virginia RN License
• Passionate and motivated to provide care within a patient's home
• Ability to exercise initiative and independent judgment
• Comfortable working collaboratively in cross-functional teams
• Ability to communicate effectively, both verbally and in writing, with all levels of an organization
• Ability and willingness to travel within a given territory
• Willingness to work with multiple patients in varying home locations

Preferred Qualifications:

• Two (2) years home health nursing experience, are preferred.
