General Responsibilities:
The nurse is responsible for performing successful telephone assessment of a patient’s condition using established guidelines/protocols (Schmitt and Thompson) under the clinical guidance/supervision of the Physician and/or NP. Contributes to the smooth delivery of excellent patient care via telephone. Develops appropriate relationships with patients, families, and colleagues as a team member providing the highest quality patient care.

Qualifications: Required Qualifications:
  • Current RI RN license
  • Ability to communicate effectively and work collaboratively with patients, providers, other members of the health care team, other disciplines, and community agencies.
  • Professionalism, self-starter, leadership skills, triage skills, documentation skills, and teaching skills
  • Experience with computers, web-based programs, EMR. Ability to interpret, adapt, and apply guidelines and procedures
  • CPR certification required
Preferred Qualifications:
  • Two or more years nursing experience preferred
  • Ambulatory Health Care experience preferred

Significant Job Functions:
  • Efficiently obtains an accurate medical history, determines chief complaint, and provides appropriate disposition of the patient.
  • Answers the caller’s questions and provides pertinent advice and education.
  • Utilizes critical thinking and problem-solving skills to recognize and match symptoms to protocol, assigning acuity (emergent, urgent, acute, and non-acute), and escalates to the provider when required.
  • Offers home treatment advice as appropriate. Accesses patient information via EMR.
  • Provides reassurance and direction to the caller.
  • Resourceful, resilient self-starter who can work within in an interdisciplinary environment.
  • Knowledge of the teaching principles of health care information to pediatric, adult, and geriatric populations.
  • Communicates based on patients’ level of understanding and comprehension.
  • Serves as the primary point of contact for telephone patients with a variety of health or illness questions.
  • Documents encounter and decision making in EMR in a concise and accurate manner, utilizing the patient's own words as appropriate.
  • Listens, communicates, and documents simultaneously and navigates computer programs to assess, process, educate and document all information in a professional manner.
  • Recognizes and incorporates the dynamic scope and standards of practice for the professional ambulatory care nurse
  • Utilizes the nursing process to facilitate decision making by performing problem focused assessments of patients’ and/or parents primary concerns, identify problem(s), analyze, and integrate subjective and objective data, decide on a plan of nursing action, apply appropriate nursing interventions, and evaluate outcomes
  • Follows laws, policies, and regulatory requirements governing healthcare services and practices in the state of Rhode Island.
Physical Effort/ Environment:
  • This role is remote (local), with onsite training expected.
  • Minimal physical activity, desk/computer use.
Work Schedule Demands:
  • Part- time to Full-time position with hours scheduled during hours of operation (8am-8pm Monday through Thursday and Friday 8am-5pm).
Communication Skills:
  • Interpreting medical terminology. Uses all available methods of communication with patients such as interpreting, counseling, group discussion, written materials, and visual aids.
  • Has sensitivity to the patient population we serve, by identifying cultural, social, and economic characteristics.
  • Able to document clearly, competently, and concisely in the medical record.
  • Ability to maintain confidentiality of information and communicate in a professional manner with patients, families other staff and outside contacts
